AFFIDAVIT OF UNAVAILABILITY OF EMPLOYMENT

CERTIFICATE DUE TO COMPANY CLOSURE

THAT I, Mary Joy Claney, Filipino citizen, Married and with

current postal address at #533 Lemary Suite Condominium, Unit
1211 Shaw Boulevard, Mandaluyong City, after being sworn in
accordance with law hereby states the following:
1. That as a pre-requisite for employment, I was required to obtain
a Certificate of Employment from my former companies;

2. That, my former employers were:

2.1 Manila Medical Philippines, Inc., located at Shaw Blvd.,
Mandaluyong City in which I work as a Medical
Transcriptionist from February 28, 2005 to February 9,
2008, and,

2.2 Philscribe Healthcare, Inc., located at #115 General Manio

St., San Pedro Village, Makati City in a capacity as
Transcriptionist/Medical Editor from March 2008 to May
2011;

3. That upon complying with the requirement for my new job, the
above-mentioned companies unknown to me were already
closed which I only discovered when I went to their office
addresses;

4. That I have performed all earnest efforts to contact the above

companies/employers, however, such efforts did not yield
positive results;

5. That as a result of their closure, I was not able to obtain a

Certificate of Employment;
That I am executing this affidavit to attest to the truthfulness of
the foregoing statements. That If I learn in the future of their
existence I would immediately inform my current employers. Further,
this affidavit is being executed to attest that there is no fraud, deceit
or any misrepresentation in order to gain advantage for purposes of
employment.
