PETITION FOR RECONSTITUTION

OF TRANSFER CERTIFICATE OF TITLE

COMES NOW, petitioner RODRIGO GUICO through counsel, unto

this Honorable Court, respectfully alleges:

1. That petitioner is of legal age, single, Filipino citizen and residing at Brgy.
San Antonio, Calapan City Oriental Mindoro; that respondent REGISTER
OF DEEDS is the Register of Deeds of Calapan City Oriental Mindoro where
he may be served with summons and other court processes;

2. That petitioner is the registered owner of a parcel of land located at BRGY.

SAN ANTONIO CALAPAN CITY, ORIENTAL MINDORO and covered by
 Transfer Certificate of Title No. T-54493, Register of Deeds of Calapan City,
free of any encumbrances;

3. That the copy of the Transfer Certificate of Title which is in the possession of
the Petitioner have been severely damaged and not readable due to old age
of the document copy of the Transfer Certificate of Title No. T-54493 is hereto
attached as Annex A;

4. That the subject property covered by TCT No. T-54493 is also covered by Tax
Declaration No. 11-046-00237 issued by the Office of the City Assessor, a
copy of which is hereto attached as Annex B;

5. That the Petitioner is continuously paying the Real Property Tax of the said
property as evidence by the receipt Provincial or City Treasurer’s Real
Property Tax Receipt No. 1380741 issued by the Provincial or City Treasurer
copy of which is hereto attached as Annex C;

4. That said transfer certificate of title was never mortgaged or sold to anyone.

WHEREFORE, it is respectfully prayed that the Register of Deeds of

Calapan City issue a reconstituted original certificate of title based on the
owner’s copy of the certificate of title hereto attached in the name of herein
petitioner.

Other just and equitable reliefs are likewise prayed for.
