 DIALOGUE DOCTOR-PATIENT

SITUATION: Caca, 19 years old was a teenager with thin, lethargic, and not excited like other
teenagers. Her father was a scavenger and her mother works as a housemaid, she had 5
siblings. She came to a family doctor not far from his house. Doctor the family asked a
history and physical examination. She hoped that the doctor could give some medicines and
advices so she could feel better.
Doctor : Good morning Ms, I am Indah Febranambela Jovie as a doctor in this clinic
for this morning. How can I call you Ms?
Caca : Good morning doctor, You can call me Caca doc
Doctor : How old are you Ms Caca?
Caca : I am 19 years old
Doctor : Oh, you go to university, don't you?
Caca : No doctor. Now, I work as an employee at one of mini market near from
here.
Doctor : Im sorry to hear that
Caca : No problem doctor. I have 5 siblings in my hause. My father is a scavenger
and her mother works as a housemaid
Doctor : Once again I'm sad to hear that. But you are a strong. I am proud of your
Business. By the way, where do you live Caca?
Caca : I live at Asra street number 8 Padang doctor
Doctor : Okay Caca, what can I do for you?
Caca : I feel lethargic and not excited like other teenagers. My body is also thin
Doctor : Oh i see, when is the symtomps appear?
Caca : Its around this 3 months
Doctor : I will check your vital sign to check your overall health. I will check you
temperature, blood pressure, pulse, and respiratory rate. You can lie down

Caca : How is the result doctor?
Doctor : Your temperature is 37 celcius, you blood pressure is 110/80 mmHg, your
pulls is 85 bpm, your breathing is 15 times per minutes
Caca : Is that normal doctor?
Doctor : Yes, that is normal. Now, I’d do some physical examination. Before that,
please be in relax (Doctor do some physical examination such as conjutival,
sclera, hepatosplenomegaly, and enlarger glands)
Caca : How is the result doctor?
Doctor : From my physical examination. I find that You are pale, lethargic, non sclera
jaundice, conjunctival anemias, no hepatosplenomegaly, no enlarged
glands
Caca : What does it mean doctor?
Doctor : I suspect , you get anemia. to ensure that you will do a laboratory
examination. I will give a reference to the laboratory and you can come back
here to see the results later
Caca : Okay doctor, thank you
Doctor : You are welcome

Caca : Doctor, this the result of my laboratory examination

Doctor : The result is Hb 5.8 g / dL, leukocytes 7,200 / m3, LED 52/1 hour, urine
within normal limits, stool does not yet exist. Calculate leukocyte type:
Caca : Hmm I don’t understand the meaning of it doctor
Doctor : From the laboratory examination, you get high anemia. I suspect you get
iron deficiency anemia
Caca : Is this dangerous doc?
Doctor : Don’t worry Ms. I will give medicine to make your condition better

 CASE PRESENTATION
A patient named Caca, female, 19 years old, an employee came with complaint
thin, lethargic, and not excited like other teenagers. From the history the doctor got
information that she was in a poor family environment. Then, doctor check Caca’s vital
signs. She had a normal blood pressure, pulse, breathing, and body temperature. On
physical examination, she had pale, lethargic, non sclera jaundice, conjunctival anemias, no
hepatosplenomegaly, no enlarged glands. Doctor suspected the diagnose is anemia. To
ensure that Caca did a laboratory examination. From laboratory examination, the result was
Hb 5.8 g / dL, leukocytes 7,200 / m3, LED 52/1 hour, urine within normal limits, stool does
not yet exist. Calculate leukocyte type: 0/5/1/64/24/6. So, the doctor concluded that Caca
got Iron deficiency anemia. The doctor gave the recipe about it. The doctor also asked Caca
to consume food that have high iron content and hope Caca to be get well soon.
