AFFIDAVIT

I, MORENA ARTURO Filipino, of legal age and with office

address at Rm. 8, 2/F, Munoz Market, Proj. 7, Quezon City , after having
been duly sworn to in accordance with law do hereby depose and say:

1. That I am currently employed as clerk at Maxiwin Manpower

Services;

2. That I have been assigned by the Owner/Manager to handle

several clients.

3. That one of the client under my watch is FILPRIDE.

4. That sometime in August I received summon from NLRC with

case No. NCR-04-06187-18.

5. That instead of giving the summon to the owner/manager Ms.

Glenna Masujer, I have inadvertently put the said summon in my
drawer.

6. That I did not noticed the same until we received an Order

declaring Maxiwin Manpower Services waived the right to submit
position paper.

7. That I am executing this affidavit to attest the veracity of the

foregoing.
