AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, TERESITA F. BUHANGIN and FERMINA F. TEJADA, Filipinos, and both residents
of District # 2 Reina Mercedes Isabela,, after having been duly sworn to in accordance with law,
hereby depose and state:

1. That we are both natives of Reina Mercedes, Isabela and residents for more than fifty
(50) years, and during this period of time, we have personally know the person of
Marilou Patriarca Lumaban, an applicant for the late registration of her birth;

2. That during the same period of time we personally know that she is among the children
of spouses Catalino Canceran Lumaban and Josefa Calimag Patriarca. She was born
on December 21, 1968 at Reina Mercedes, Province of Isabela, Philippines;

3. That we came to know the fact that based on the NSO letter dated November 21, 1995,
it was certified the negative result of Marilou’s birth record. It further certified that her
birth record was among the destroyed records on file in the Local Civil Registry of Reina
Mercedes, Isabela;

4. At, present, Marilou Patriarca Lumaban is married to FEDERICO C. REGALADO, who

also personally known to us, thusly, in all her present undertakings, she has been using
MARILOU LUMABAN REGALADO. The two were blessed with three children namely:
Ruth Syvill Joyce L. Regalado - 20 yrs old, Hannah Kristel L Regalado – 16 years old,
and Psalm Gabriel L Regalado – 14 years old;

5. In view thereof, we would like to affirm and declare that MARILOU PATRIARCA
LUMABAN the above named applicant for late registration of birth is the same MARILOU
LUMABAN REGALADO as appearing in all her present undertakings;

6. That we knew the above stated facts because we are her former neighbor, and
TERESITA F. BUHANGIN, one of the undersigned, was the midwife (now retired) who
attended Marilou’s birth;

7. That we are executing this joint affidavit to attest to the truth and veracity of the
foregoing set of facts and for other legal intents and purposes.

FURTHER AFFIANTS SAYETH NONE.
