Pinukpuk Kalinga, after having been duly sworn to in accordance with law, do
hereby depose and state that:

1. ANNIE LAURIE DUMPAO JOVEN was born to parents VICTORIA

DUMPAO and TEODORO GONGOB of Aciga, Pinukpuk, Kalinga;

2. ANNIE LAURIE DUMPAO JOVEN was in our care since she was a baby
and me and my wife considered her as our youngest child;

3. We have enrolled ANNIE LAURIE DUMPAO JOVEN in school using the

said name;

4. School records shows ANNIE LAURIE DUMPAO JOVEN as her name

contrary to what is indicated in her Birth Certificate as ANNIE LAURIE
DUMPAO GONGOB;

5. ANNIE LAURIE DUMPAO JOVEN appearing in the school records and

ANNIE LAURIE DUMPAO GONGOB in the Certificate of Live Birth
pertains to one and the same person;

6. I am executing this affidavit to attest to the truthfulness of the above facts

and for whatever legal purposes and intents it may serve.
