AFFIDAVIT OF SUPPORT AND CONSENT

I, CRISTINA G. BAUTISTA, Filipino, of legal age and residing at 12 Kent

St., Filinvest 2, Batasan Hills, Quezon City, state under oath:

1. That I am the mother of JUSTINE THANIA G. BAUTISTA who was born

on September 17, 1999;
2. That my daughter applied for Visa Application as tourist before the Korean
Embassy in the Philippines;
3. That I am fully giving my consent to my daughter to travel to South Korea
as tourist;
4. That I have sufficient income/assets to responsibly cover my daughter’s
necessary expenses for the duration her travel to South Korea as well as any
unforeseen expenses which my daughter may incur;
5. That I hereby accept and assume any and all responsibility for her welfare,
including all financial expenses related to the said travel;
6. That I guarantee that my daughter will abide by all laws and regulations of
the government of South Korea;
7. That I am executing this affidavit to attest to the truth of the foregoing facts
and in support of my daughter’s Visa Application.
