JOINT AFFIDAVIT
(As required by Article 34 of the Family Code
Of the Philippines as Amended by Executive
Order No. 277, July 17, 1987)

That we, JOERITA TAMAYO SULLIVAN and HENRY DURAN CASTILLO, both of legal
age, Filipinos, Single, and residents of barangay Bunao, Dumaguete, Negros Oriental,
after having been duly sworn to in accordance with law, do hereby depose and
manifest:

That we met each other sometime in 1993 in Bunao, Dumaguete City because we
were neighbors at that time, then we became acquainted with each other and
subsequently, we became intimate friends;

That moved by our love and mutual understanding, in the year 2001, we decided
and started living together as husband and wife in Bunao, Dumaguete City until the
present;

That during our cohabitation, we were blessed with four (4) children, namely:
HAROLD KENT T. SULLIVAN, who was born in August 16, 2001; BRYAN KIT T. SULLIVAN,
who was born in October 11, 2002; ADRIAN KYLE SULLIVAN CASTILLO, who was born in
October 18, 2008; PRECIOUS SHANE SULLIVAN, who was born in November 10, 2012;
and ANDIE, who was born in August 13, 2013;

That we have been living together as husband and wife for more than (17) years
already without the benefits of being legally married and it is now our desire to marry
each other for our own good and welfare of our children;

That there is no legal impediment to our marriage as we both possess the legal
capacity to marry each other, we being single and of marriageable age;

That we executed this affidavit freely and voluntarily just to attest to the
truthfulness and veracity of our statements herein;

IN WITNESS WHEREOF, we have hereunto affixed our signatures this ___ day of
August 2018, at Dumaguete City, Negros Oriental.
