JOINT AFFIDAVIT OF COHABITATION

(As required by Article 34 of the Family Code
Of the Philippines as Amended by Executive
Order No. 277, July 17, 1987)

That we, JESUS L. VERUEN JR. and JOENALE Q. FORTIN, both of legal age,
single, Filipinos, and residents of barangay Maslog, Sibulan, Negros Oriental, after
having been duly sworn to in accordance with law, do hereby depose and manifest:

That we met each other sometime in 2008 in a festival in the barangay in which
we are both residents and then later on, we became intimate friends;

That moved by our love and mutual understanding, in the year 2009, we decided
and started living together as husband and wife in Maslog, Sibulan until the present;

That during our cohabitation we were blessed with one child named, Sol Jade F.
Veruen, who was born in November 9, 2012.

That we have been living together as husband and wife for more than Five (5)
years already without the benefits of being legally married and it is now our desire to
marry each other for our own good and welfare of our future children;

That there is no legal impediment to our marriage as we both possess the legal
capacity to marry each other, we being single and of marriageable age;

That we executed this affidavit freely and voluntarily just to attest to the
truthfulness and veracity of our statements herein;

IN WITNESS WHEREOF, we have hereunto affixed our signatures this 26th day
of June 2018, at Tayasan, Negros Oriental.
