Lessons Learned from Incidents during 2012

I ncident is an event or chain of events which has caused or could have cause a fatality,
injury, or illness or damage(loss) to assets , environment , entity reputation or third parties.

Incident Occurred during 2012


are Mainly due to:

 Vehicle equipment &truck


overturn.
 Falling from height.
 Contact with moving parts.
 Unsafe scaffolds.
 Unsafe electrical connections.
 Unsafe excavations.
 Unsafe lifting operations.
 Fires.
 Hit by objects.
 Slip trip & fall.
Incident Prevention Techniques:

 Highlight high cost of incidents.


 Proactive Performance
Measurement(JSO,JSA & use checklist
always).
 Comply with HSE rules & regulations.
 Management should commit to HSE
& set the right example.
 Increase the awareness through training.
 Recognize the standards & the best
practices & implement them.
 Promote EHS incentive program.
 Study & investigate near misses.
 Always assess the risk of your activities
& implement the control measures.

Two Lives Lost due to an Incident where
Windows Cleaning Cradle Fell from the 14th
Floor:

 No any measures implemented to


prevent this incident.
 Incompetent staff deployed to release
the stuck cradle.
 Major safety device was removed by
the team.
 Platform was not tested and certified.
 Risk assessment &hazard were not
identified and documented.
 Emergency Procedure was not
prepared and applied.
 Lack of Awareness.
 External rescue teams were not
summoned to rescue the trapped
employees. Lesson Learned & Strict Action:

 All companies involved in facade


cleaning instructed to prove their
competency & register with ADM
HSE Division.
 All equipment shall be certified with
3rd party to prove safe erection &
conditions for use.
 Companies shall ensure training &
competency of their employees
 Permit should be obtained from ADM
building permit.
 Ensure preparedness for
emergencies.

A Foreman had been Buried Alive when the When Excavating:
Soil Settled under his Feet:
 Specific method statement &Risk
assessment shall be prepared.
 All required NOCs should be obtained

Main Direct Causes of the 2012:

 vehicle and equipment prior to each
use.
 Determine the speed limit in the
construction site.
 Deploy Banksmen and observe blind
corners.
 Segregate pedestrians & vehicles.
 Ensure dust control measures when
necessary.
 Ensure a competent person inspects.
 Ensure activities are adequately
supervised.

Falling from Height



Almost 50% of fatal incidents in construction sites attributed to falling from height, to
avoid risk of fall all works at height should be properly planned and adequately supervised
