Pursuant to Rule 5, Section C, Chapter II, Part IV of the NEUST Student Manual of
2001, we hereby signify our intention to renew the recognition of our Non-Political Student
Organization,

a. Formal letter of intent to seek renewal for recognition;

b. Updated Constitution-and-by-Laws if amended, (if not amended, only a certification to
this effect signed by its President and concurred by the Faculty Adviser);
c. Updated list of Board of Officers, with their names, respective positions, course and
year, and address;
d. Name of the Faculty Adviser/s with a Letter of Acceptance addressed to the Dean of
Student Affairs;
e. Proposed activities for the entire school year including the tentative dates of
implementation and brief description of the activity;
f. List of at least 30 members with their names, course and year, addresses and specimen
signatures.

_______ or until a written resignation as such is filed to the Office of the Dean of Student Affairs
and Head, Office of Student Organizations, Activities and Development or be removed for a cause.

As ADVISER, I commit and bind myself to render guidance to the Officers and members
especially in the aspect of their physical, intellectual, social, moral/spiritual and emotional
development.

Further, as ADVISER, together with the school and its Administrator, we have special
parental authority over the members of the organization/club and therefore principally liable for any
damages caused by the acts and omissions made by them in accordance with the law.

Finally, I obligate and bind myself to observe all the diligence of a good mother or father of
a family in the exercise of the duties and responsibilities as ADVISER.
