I. CONTEXT AND RATIONALE

a. Introduction (defining the system)
This entails describing the current practices observed in the institution.
b. Background (assessing the situation)
This entails assessing practices and identifying non-satisfactory performances
that requires immediate attention.
c. Identifying the Root Causes
This entails identifying factors/root causes of such non-satisfactory performances
and answers question: “What could be done differently to get a better performance?” as
introductory for the proposed innovation/intervention.
II. ACTION RESEARCH QUESTION/S
The research question/s is/are goals or aims that will be answered after the entire action
research.

the necessary steps to carry out the action.
Note: Improvement theory is the main theory to use during the implementation.
Action research is all about studying for the improvement of a specific teaching
practice for better student learning.
IV. ACTION RESEARCH METHODS
This consists the research design used, methods, participants involved, research
instruments, data gathering procedure and data analysis, and ethical considerations.
a. Participants and/or other sources of data and information
This includes the participants of the study and other sources of data and information (ex.
Your own self-reflection or observation/ teacher’s journals/ parent’s involvement, etc)
b. Data gathering methods
c. Data analysis plan
V. ACTION RESEARCH WORKPLAN AND TIMELINE
a. Pre-implementation
b. Implementation
c. Post-implementation
VI.FINDINGS
a. Results and Discussion
This comprises the result of the study and answers the research question/s: “Did my
improvement theory work? What are the changes occurred during the
implementation? How has my professional planning changed? How will I sustain
these changes?”
VII. ACTION RESEARCH SUMMARY, CONCLUSION, AND RECOMMENDATION
This consists the summary, conclusion and recommendation of study.
VIII. ACTION RESEARCH WORK PLAN AND TIMELINES
a. GANTT Chart
IX. PLANS FOR DISSEMINATION
What are your plans for dissemination? EX: attend local/school-based research
conference, etc.
