depose and state that:

1. I am applying for Senior High School teaching position at Misamis Occidental

Division.

2. I have submitted the following documents:

a) Letter of Intent
b) CSC Form 212 in two(2) copies with the latest 2x2 ID picture.
c) Certified photocopy of certificates of relevant specialized trainings.
d) Certified copy of Voter’s ID and/ or any proof of residency.
e) National Bureau of Investigation Certificate.
f) Omnibus certification of authenticity and veracity of all documents submitted,
signed by the applicant.
g) Certified photocopy of Diploma on Bachelor’s Degree.
h) Certified photocopy of Transcript of Records.
i) Certified photocopy of Professional Regulation Commission (PRC) professional
ID card.
j) Certified photocopy of ratings obtained in the Licensure Examination for
Teachers (LET).

3. I am executing this Certification to attest to the authenticity and veracity of all

documents submitted

4. By executing this Omnibus Certification of Authenticity and Veracity of Documents, I

hereby authorize the Department of Education, Misamis Occidental Division through the
School Principal, to verify the authenticity of the abovementioned documents.
