AFFIDAVIT OF WITNESS

I, Rose Hernandez, of legal age, married, and a resident of 1905 Street,

Gangnam Condominium, Binondo, Manila after duly sworn to in accordance of
law hereby depose and state:

1. That I am an employee at Chuva Chenes Logistics Services as the operation

manager;

2. That on 26 May 2018, I was assigned with Lalisa Manoban to attend the two
day meeting at Subic with one of our clients to present the offer of our
company and pay them the initial deposit for the transaction;

3. That Lalisa Manoban was unable to attend with me because she was on
leave due to her injuries, that is why she asked the respondent if she could
come with me on behalf of her;

4. That I called the respondent to hurry up and bring with her the ONE
HUNDRED THOUSAND PESOS for the initial deposit;

5. That when she arrived, I handed her the billing statement of the client;

6. That she was with me until 28 May 2018 for the said two day meeting in
Subic with our client;

7. That I executed this affidavit to attest the truthfulness of the foregoing facts
and to support the filing of criminal case against Matilda Matias De Leon,
for Qualified Theft.
