Pre employment medical Test of the employees @ Rs 640.00 per person will be done as per the
agreement of both the parties as follows up till 31-03-2020: Payments to be made either by
cheque or cash.

1. General Examination :
a. Height b. Weight c. Chest d. Blood Pressure e. Pulse f. BMI (Body mass Index)

2. EYE : Right Eye, Left Eye, Near Vision, Far Vision, Color Vision

3. Blood Test : Total RBC, Total WBC, Blood Group, Haemoglobin

4. Urine Examination : Physical, Chemical, Microscopic Examination.

5. X-ray (Chest)

6. ECG

In Consideration of the matters described above and of the mutual benefits and obligations set
forth in this Agreement, the receipt and sufficiency of which consideration is hereby
acknowledged, the Customer and the Service Provider (individually the “Party” and collectively
the “Parties” to this Agreement) agree as follows:
