AFFIDAVIT

I, ANGELES GUANZON PUNSALAN, of legal age, Filipino, with postal

address at No. No. 33-B Sct. Dr. Lazcano Street, Brgy. Laging Handa, Quezon
City, Philippines, after being duly sworn in accordance with law, do hereby
depose and state that:

1. I was born on 17 April 1934 in Binikuli Kabankalan, Negros

Occidental, Philippines;

2. I was able to secure a certified copy of my Birth Certificate

correctly showing that my date of birth is 17 April 1934. In fact, I used my
correct Birth Certificate, showing my date of birth on 17 April 1934, which was
used and became the basis for the issuance of my Philippine Passport showing
my date of birth as 17 April 1934. My correct Birth Certificate was also used
and became the basis in securing my Permanent Resident Card from the
United States of America which correctly indicated my date of birth as 17 April
1934. A copy of my Philippine Passport and Permanent Resident Card are
attached hereto as Annexes “A” and “B” and made integral parts hereof.

3. However, it would appear from the records of the National

Statistics Office (“NSO”) that I have another Birth Certificate indicating my date
of birth as 17 April 1941. I am hereby confirming that my real and actual date
of birth is 17 April 1934 and not 17 April 1941. Therefore, the Birth Certificate
showing my date of birth as 17 April 1941 is clearly erroneous and should be
expunged from the records of the NSO.

4. In fact, as added proof that I was born on 17 April 1934, in the

Birth Certificate of my son Rex Guanzon Punsalan, born on 6 September 1968,
it correctly showed my age at his birth as 34. Subtracting my age then at 34
from the year 1968, it shows that I was born in 1934. A copy of the Birth
Certificate of my son Rex Guanzon Punsalan is attached hereto as Annex “C”
and made an integral part hereof.

5. In addition, in the Birth Certificate of my daughter Ruby Guanzon

Punsalan, born on 12 July 1973, it correctly showed my age at her birth as 39.
Subtracting my age then at 39 from the year 1973, shows that I was born in
1934. A copy of the Birth Certificate of my daughter Ruby Guanzon Punsalan
is attached hereto as Annex “D” and made an integral part hereof.

6. I am executing this affidavit to attest to the truth of the foregoing

facts and for the NSO to expunge from their records my erroneous birth
certificate showing that I was born on 17 April 1941 and for whatever legal
purposes it may serve.
