Congratulations! You passed the 2019 DOST-SEI Undergraduate Scholarship Examination which you took last 21 October 2018
and hence, you qualified for the 2019 DOST-SEI Merit Scholarships.

To be able to enjoy the scholarship, you are to pursue a 4- or 5-year Bachelor of Science (BS) degree program in any of the priority
Science and Technology(S&T) fields of study at the University of the Philippines System, state university or college, or any private
higher education institution that is recognized by Commission on Higher Education as Center of Excellence or Center of Development
or with FAAP Level III accreditation for the BS program that you intend to enroll in. For the complete list of priority S&T degree

In this regard, you, together with one (1) parent only or a legal guardian, are advised to report on the following occasion:

Orientation on S&T Scholarship Policies and Signing of Scholarship Agreement

Venue: Reception Hall, Philippine International Convention Center (PICC)

Address: Pasay City, Metro Manila
Date & Time: 21 May 2019, 7:30 AM

There shall be strictly one (1) general session for the orientation on the indicated schedule. Thus, please come early and bring the
following requirements:

1. Photocopy of 2019 Community Tax Certificate or any government-issued ID (e.g. valid passport, GSIS ID,
postal ID, etc) of Parent/Guardian;
2. Notarized affidavit of guardianship or special power of attorney (if legal guardian); and
3. Black ballpen

Upon receipt of this notice, please confirm immediately your attendance by using either of the following subject line:

1. ATTENDING
2. NOT ATTENDING

Indicate your control number (seen at the upper right portion of this notice), full name, and scholarship program.
