PROPOSAL FORM OF

Gentlemen
In accordance with your invitation to bid, we hereby submit our proposal subject to all conditions and
requirements of the plan and specifications as prepared by AZ Architectural Planners, Inc. Unit 1507, 15/F
2210 Cityland Pasong Tamo Tower, Don Chino Roces Ave. Brgy. Pio del Pilar, Makati City and other related
documents for the above mentioned project.
We promise to furnish, deliver, place and complete any and all necessary materials, labor, appliance,
tools and equipment, supplies, utilities, transportation, supervision and other facilities in connection therewith
and to perform all operations for the complete scope of works at our own expense in the lumpsum amount of
PESOS : *** FOUR MILLION ONE HUNDRED FIFTY THOUSAND PESOS ONLY ***
( PHP 4,150,000.00 )
We understand that the Owner reserves the right to order additional work or to reduce the scope of
work if deemed necessary. All extra work shall be paid for in accordance with the Bid Schedule included in
this proposal.
Should we may be required to perform work over and above that required by the Contract Documents,
payments shall be based on bare cost of materials and labor plus mark-up per contract.
Prices of materials shall be based on bid unit market prices at the time the work order is made by
the Owner.
Should the current minimum wage be increased by legislation during the pendency of or any time
during the same as that indicated in the Bid Proposal.
Should we be ordered to omit / delete work required by the Contract Documents, we shall credit
the Owner, payments based on the bare cost of materials and labor plus mark-up.
We make this proposal with full knowledge of the kind, quantity and quality of the articles and
services required, and if said Proposal is accepted, we will enter into a contract with good sufficient
securities for the fruitful performance thereof.
We understand that the Owner reserves the right to reject any or all bids and to waive any defects
or informality in the bids received and that the Owner may accept any item or group of items of any bid,
unless it is so disqualified by specific limitations.
We will complete the work in accordance with the plans, specifications and other related Contract
Documents within Thirty ( 30 ) CALENDAR DAYS (Sundays and legal holidays included) reckoned
from the date as stipulated in the Notice to Proceed. No request for the extension of time will be
made other than that due circumstances of "force majeure". We understand that time is an essential part
of this proposed Contract and upon failure to complete the work within the stipulated time, we agree to
pay liquidated damages per calendar day of delay as stipulated in the contract.
We certify that we have prepared and or checked and reviewed the contract procedures, contract
schedule, equipment requirements and price submitted with this Proposal.
In the event that we are awarded with this project, we will be assigning a licensed architect or
civil engineer at the site as a full time Project Architect / Engineer and a registered electrical engineer as
a full time Project Electrical Engineer.
We confirm the receipt and consideration of the following bid bulletins prior to the submittal of our
proposal. We have included the work called for in this bid bulletins in our proposed price.
