RESOLUTION CRAFTING SUPPLEMENTAL ANNUAL PROCUREMENT PROGRAM FOR THE

ADDITIONAL MOOE SUBSIDY FOR SAN VICENTE NATIONAL HIGH SCHOOL – SENIOR
HIGH SCHOOL.

WHEREAS, the Additional School MOOE is based on the computation of the actual
enrolment of the senior high school to provide additional instructional materials and other
operational expenses.

Whereas, the Bids and Awards Committee of San Vicente National High School has
agreed to issue recommendation to the School Head Mr. RICHMOND L. SUMAGANG
through the BAC Secretariat to craft supplemental annual procurement program for San
Vicente National High School-SHS.

WHEREAS, upon examination of the additional fund, it was divided to governance, quality
and access expenses in the annual procurement plan.

NOW THEREFORE, upon motion of Mrs. Annabelle B. Subrado and duly seconded
by all present.

BE IT RESOLVED, AS IT IS HEREBY RESOLVED that the BAC recommends the

School Head Mr. Richmond L. Sumagang that shopping be applied as the mode of
procurement for the above mentioned procurement.

RESOLVED further that copies of this resolution be furnished to the Office of the
School Principal and the State Auditor IV for DepED Bukidnon further perusal and
appropriate action.

RESOLUTION RECOMMENDING SHOPPING AS ALTERNATIVE METHOD OF

PROCUREMENT OF THE ATTACHED PURCHASE REQUEST

Whereas, the Bids and Awards Committee of San Vicente National High School has
agreed to issue recommendation to the School Head Mr. RICHMOND L. SUMAGANG
regarding the mode of Procurement of Construction Supplies for Repair and
Maintenance of San Vicente National High School-JHS.

WHEREAS, upon examination of said purchase requests and the supporting documents
required for the procurement, the BAC Members find that the purchase was in accordance
with the provision of Revised IRR Rule XVI Section 52a of Revised IRR of R.A 9184
(effective September 2, 2009).
1. Unforeseen contingency requiring immediate purchase.
2. Goods procured are according to the approved budget and included in Annual
Procurement Plan (APP);
3. Funds for the said procurement will be charge to MOOE funds;
4. The prices offered by the supplier are most advantageous to the government.

NOW THEREFORE, upon motion of Mrs. Annabelle B. Subrado and duly seconded
by all present.
