I. Introduction
As stipulated in Dep. Ed Order No. 31, s. 2012, the K to 12 Basic Education Program shall start
its first year of implementation school year 2012 - 2013. It also includes the general guidelines that show
the design of Grade 1 to 10 Curriculum. The new curriculum features several changes other than an extension
of two ( 2 ) more years for Senior High School like the use of curriculum guides, teaching guides and
more so in its grading system.
The grading system was thoroughly discussed and explained by Dep. Ed. Order No. 73: "Assessment
and Rating Student's Learning Outcome". Grading the students at the end of instruction is the attainment
of learning outcomes defined by curriculum standards which sets an assurance of learning through summative
assessment. Changes lie entirely on the definition of the learning outcomes based on four (4)levels of assessment:
Knowledge - 15%
Process/ Skills - 25%
Understanding - 30%
Product/Performance - 30%
Aside from those changes in the computation of grades, the process of describing grades in the
student's report cards is in terms of the four levels of proficiency with its equivalent numerical value:
These particular changes brought some confusion and require adjustments to the teachers
because of the different levels of assessment which is entirely new to them plus the use
of various level of profiency in every numerical grade as they rate student's academic performance.
New Grading System requires new ways of filling up forms thus we have the new set of Modified
School Forms to accomplish this school year 2014 - 2015.
This innovative material, Automated Forms for Secondary School Teachers, was intended for the use
of teachers and class advisers of Cararayan National High School. The proponent, MARIA TERESITA R. RENTOY,
will conduct a seminar workshop to orient all CNHS teachers in K to 12 BEC and RBEC. Although this has been
adopted by Grade VII teachers last school year 2013 - 2014, they will also need to attend the same seminar
workshop for there are some specific instructions that they still need to know. Each teachers should save the
program in their laptop, flash drive or other gadgets that may save the said program.

An Innovation: Automated Forms for Secondary School Teachers is a soft copy design of school
forms using Microsoft Excel. It was created by the proponent meant to provide easy access and completion
of various school forms. It can be saved in any data storage devices like CD, DVD, USB flash drives or can
be accessed online through the proponent's Facebook or Yahoo mail account.
This innovative material comes in three different workbooks with file names:

Formatted Modified School Forms.xlsx
Each saved workbook contains a Table of Contents , Introduction and Instruction that gives an
information about each saved workbook and the step by step process of using the workbook and its
corresponding saved worksheets in it. Each pages contain the following columns:

Table of Contents which contains worksheet names of this workbook. All you have to do is to click
the name of the needed worksheet and it will automatically link in the specific

Formatted School Form 1 (SF1) This worksheet is SF 1: School Register which is to be accomplished BoSY and EoSY.
It is automatically linked with the other worksheets so follow the reminders below:
Reminders: 1 Encode the following data ONCE in this worksheet: school ID, school name, region,
division, district , school year , grade level and section inside the box.
2 Fill up the following columns that are also linked to other worksheets: No. , LRN and
name of students.
3 Fill up the remaining columns with appropriate informations but don't forget to update the
REMARKS column before the EoSY. ( Pls. refer to the list and codes of indicators for Remarks)

Formatted School Form 2 (SF2) This worksheet is SF 2: Daily Attendance Report of Learners which is to be accomplished
daily and submittedon or before 5th day of every month. Since it is formatted, there are
some informations that are linked to the previous worksheet. Please follow the reminders
below in filling up this worksheet.
Reminders: 1 Fill up ONLY the box "Report for the month of" , "date and day" and column of "No."
2 Put an "\" marked once the students is absent. Tardy mark , type comma: ","
while Cutting Classes mark ,type point ".". It will automatically shade the correct region.
3 Encode "DRP" for dropped in AE Column , "T/O" for Transferred Out in AF Column and "T/I"
in AG Column . (Please refer to the legend below the form for other needed informations)
