INSPECTION CHECKLIST –SCAFFOLDING

1. Is the footing sound and rigid-not set on soft ground or resting on blocks?
2. Has the erection site been evaluated for hazards such as earth fills, ditches, debris,
underground electric wires, unguarded openings or conditions created by other
trades?
3. Are the bases of scaffolds at least 1.5 times the depth of excavation away from edges
of excavation (including trenches), unless adequate precautionary measures are taken
to prevent the collapse of the excavation and ensure the integrity of the scaffold
foundation?
4. Is the scaffold able to hold four times its maximum intended load?
5. Are guardrails and toe boards in place on all open sides?
6. Is the platform complete front to back and side to side (fully planked or decked, with
no gaps greater than 1 inch?
7. Is the lumber free of cracks, splits, knots, or damage?
8. Is the scaffold level?
9. Have all components been inspected for defects such as broken welds, corroded
members, and missing locks, bent or dented tubes?
10. Are all braces, bearer, and clamp sections pinned or appropriately secured?
11. Is the front of the scaffold within 14 inches of the work?
12. Does the scaffold meet electrical safety clearance distances?
13. Is the scaffold less than 125 feet in height?
14. Is the “X” bracing installed on the ends of the scaffold and every third set of post
horizontally and every fourth vertical runner?
15. Have all planks been properly secured to the scaffold structure to prevent them
blowing off in the event of high winds?
16. Where persons work under scaffold, is a ½ inch mesh screen provided between toe
board and guard rail or has the area below the scaffold been cordoned off?
17. Are tag lines available for items to be loaded on to scaffold?
18. Is the scaffold over 1.8 meters (6 feet) high; (if yes) are fall protection systems
(personal fall arrest/guard rails) available in place?
19. When employees are working on suspended scaffolds, are lifelines firmly anchored to
an overhead structure and not to the scaffold?
20. Are top rails between 38 to 45 inches high and mid rails in place?
21. Are toe boards in place and at least 4 inches high?
22. Does the scaffold have a height to base ratio of at least 4:1?
23. Is the scaffold restrained from tipping by ties, guys, outrigger frames over 4:1 height
to base ratio?
24. Are castor wheels locked and free from damage?
25. Are safe access provided to all working platforms by ladders, stairs/stairways, ramps,
or walkways (without climbing on cross braces?
26. Are ladders placed at a slope of 4:1 (75o) and side rails are rigidly attached to a
supporting structure at the top, middle and bottom of the ladder?
27. Are ladders extended at least 900mm (3feet) above the landing platform?
28. Are the scaffolds blocking the fire protection equipments, electrical panels, eye wash
station, first aid station and passageways during and after the erection of the
scaffolds?
