INTRODUCTION: As a post of Community Health Field Posting, I was posted in Primary Health

centre, Bhatagaon, Raipur, during this posting one of our objective was to gain in-sight to develop
and implement Individual Health and Nursing Care Plan

Growth and development distinguish child from an adult and it is essential feature of life of child. For
early identification of child’s malnutrition and inadequate health or any illness measuring growth and
development is essential part of child care. During clinical experience I select one infant to give
comprehensive care.

 Liquid semisolid foods like Biscuit soaked in milk, mashed banana, mashed and boiled potato, etc.
 Khichri, egg yolk and mixture of rice and dal.

OBJECTIVES:
 To introduce self and explain the purpose of family visit.
 To establish interpersonal relationship with the family.
OBSERVATION
After maintaining IPR, information collected regarding family and infant which are selected for care:
Temperature : 370 C
Pulse : 110/ minute
Volume : good

ADVICE
 Advised mother to keep infant clean and home also.
 Advised mother for breast feeding.
 Proper position of breast feeding.
 Advised burping after breast feeding.

2ND VISIT

OBJECTIVES:

 To collect information about family and about infant in detail.


 To assess the growth and development in detail.
 To provide need based education.

OBSERVATION:

Temperature: 370 C

Pulse: 112/min.

Volume: Good

Complete growth and development assessment of infant is done.

Height (length): 60 cm

Head (length): 43cm


Cheats circumference: 43cm

Weight: 7 kg

ADVICE

 Bring the infant for remained immunization.


 Advise for cleanliness of breast before and after feeding.
 Instruct do not apply kajal to eye.

OBJECTIVES

 To explain about prevention of minor ailments in infant.


 To provide need based health education.

OBSERVATION:

Infant has common cold:

Temperature: 37.20 C

Pulse: 114/min

Volume: good

ADVICE
 Explain about prevention of minor ailment among infant as fever, cough, cold, diarrhoea.
 Instruct to wear worm clothes to infant
 Advised mother if fever and cold increase immediately bring the child to the hospital.

4TH VISIT
OBJECTIVES
 To educate mother about proper child care
 To provide follow up
 To provide health education.
OBSERVATION
Temperature: 370 C
Pulse: 110/min
Volume: good
 Infant’s condition was improved coughing, sneezing and watering from nose decreased.
 Infant seen some active.
ADVICE
 Advised to maintain personal hygiene of infant E.g. Bathing, Wearing clean cloths etc.
 Advise to check repeatedly nappy, to prevent wetness and prevent infant
 Advised for remaining immunization.
