AFFIDAVIT OF DISCREPANCY

I, MAY ANN SISON TABUYOC, of legal age, married, Filipino and a resident of 2384
Mabini Extension, Teacher’s Village, Poblacion Zone III, Digos City, Davao del Sur, Philippines,
having been duly sworn in accordance with law, hereby depose and state:

That in my Certificate of Live Birth as recorded in the Office of the Civil Registrar of
Bansalan, Davao del Sur, Philippines and in the Philippine Statistics Authority, it is stated that I
was born on 9th of May 1977 at Bansalan, Davao del Sur, Philippines;

That my parents had used 8th May, 1977 as my birth date in all pertinent school records
up until I finished my Undergraduate Studies at Cor Jesu College of Digos, Digos City,
Philippines;

That when I took the Civil Service Career Service Sub Professional and Civil Service
Career Service Professional eligibilities on 18 February, 1996 and 19 October, 1997 respectively,
I had written the date of my birth as 8 May 1977 instead of 9 May 1977, following the date of
birth of what I used to fill in my school records;

That when I filled up my Personal Data Sheet upon entry to the government service at
Land Bank of the Philippines on March 15, 2002, I also inadvertently and erroneously written
the date of my birth as 8 May 1977 instead of 9 May 1977;

That I am executing this Affidavit of Discrepancy to attest to the truth of the foregoing
facts and to explain the discrepancy in my Certificates of Live Birth, Personal Data Sheet and in
the Career Service Professional and Career Service Sub-Professional eligibilities as regards to my
date of birth;

Further, I am declaring that from this date hereon I would be using my true and correct
date of birth of 9 May 1977 in all my papers, records and other documents.
