COMES NOW the petitioner, by the undersigned Counsel, and unto this

Honorable Court, most respectfully alleges:0

1. That the petitioner __________, married to ____________ (single) is of legal

age, Filipino citizen and a resident of ________________ is the registered

owner of a parcel of land located at ________ , municipality of __________

and province of ____________;

2. That on ______________, the Register of Deeds of ________________,

Philippines, issued Transfer Certificate of Title No. ____________________

naming the petitioner as the registered owner of a certain parcel of land

located in______________ , Philippines, more particularly described to wit:

A Certified True copy of the Transfer Certificate Title No. T-146706 is

hereto attached as Annex “A”

 3. That the Owner’s Duplicate of Transfer Certificate of Title was lost

(destroyed) and can no longer be found at its place of safekeeping and is

missing despite of serious efforts to locate the same;

4. That the said titled has not been delivered to any person or entity to secure the

payment or the performance of any obligation whatsoever, nor any transaction

or document relating to the same presented for or pending registration in the

office of the Register of Deeds of ______________.

WHEREFORE, pursuant to law, it is respectfully prayed that, after due

proceedings, an order be entered:

1. Declaring the Owner’s Duplicate of Transfer Certificate of Title No.

__________ that had been lost as null and void; and

2. Directing the Register of Deeds of _____________ to issue a new

Owner’s Duplicate of Transfer Certificate of Title the same to be

regarded as the lost original certificate for all purposes of the
