1.0 OBJECTIVE

The objective of this Standard is to standardize on the compilation, structure,


handover requirements of Project Dossiers including Hard and Soft Copy (PDF) files.
Compliance to this Standard should allow easy access to all subjects related to
Project Data records contained in the completed Project Dossier.

2.0 SCOPE

This Standard defines the (minimum) contents, structure and compilation of the Hard
Copy Project Dossier. It provides the steps to be complied with, for the approval
process (See 13.0 & 14.0) and final handover* of the Project Dossier including PDF
Soft Copies to QP Engineering Design, Info & Standards Division (PTD). Further, this
Standard defines the Data Media types, their handling and the directory structure for
Handover to QP.

* Note: Handover requirements of Project Dossiers including quantities and Sections


required by the relevant asset holders/operations, shall be addressed by the
applicable Project Team in coordination with relevant asset holder at start or early
stage of the Project, if not already defined in the Contract SOW.

This Standard DOES NOT cover the following:

 Requirements of Project Engineering Native files and their handover. This is


covered in Engineering Standard Doc No. ES.0.06.0015 Development of
Technical Drawings.

 Project Dossier Compilation for QP Refinery EPIC and Construction related


Projects. This is covered in QP Refinery Specification Document Number
2500-00-69-00-101.

The complete handover of the Project Dossiers (see 14.0) consists of:

A) Soft copy Submission of Project Dossier - PDF files


B) Completed Metadata Template (Form ES.0.08.0016)
C) Equipment Tag Key List (Form ES.0.08.0017)
D) Hard Copy Submission of Project Dossier (Original Wet Signed Approval as
applicable)

3.0 CUSTODIAN

The custodian of this Standard is Manager, Engineering Design, Info & Standards
(PTD), who is responsible for the accuracy and the quality of its contents and future
revisions, where these are required to reflect industry trends and changes to QP
business practices.

4.0 APPLICATION

With exception of QP Refinery (See 2.0) this Standard applies to all QP Contracts
including Construction, EPIC, ECS Contracts. This Standard shall also be applied to
all Sub-Contractors, Suppliers and Manufacturers.

Strict compliance with this Standard is required. Any exceptions or deviations from this
Standard, along with their merits and justification, shall be brought to the attention of
the custodian (See Section 3.0) for review, consideration and amendment as required
by completing part 1 of form ES.0.08.0014. Sample of this form is shown in
APPENDIX 11. The form is available on PT Intranet Website.

5.0 DEFINITIONS AND ABREVIATIONS

5.1 Definitions

For the purpose of this Standard, the following definitions apply:

As-Built : As-Built is the term used to describe Data that reflects the
actual plant, platform or facility status at the date shown in
the revision description that states “As-Built”. This stage is
normally completed after construction and commissioning
stage of the Project.
Commissioning : The testing of complete systems under operational or
simulated operational conditions in order to bring the
hardware to a state of ‘Ready to run’.
Data Media : The material on which Data can be recorded and can be
retrieved.
Document : A piece of written, printed, or electronic matter that provides
information or evidence.
Hard Copy : Typically refers to a printout of a Document or Drawing. In
the context of this standard, the latest wet signed approved
version.
Index : Detailed lists describing the contents and specific locations
of Data.
May : Term used to indicate a course of action permissible without
the limits of the Standard.
Native File : An electronic file that is editable in its original format of the
base (container) software.
Project Dossier : A collection of vital Project Data containing historical records
showing the items/activities performed for the duration &
completion of the Project e.g. Design Book, Mechanical
Catalogues, Equipment Lists, Drawings, Specifications, Data
sheets, Test Reports, installation & operations and
maintenance (IOM), As – Built drawings etc., and relevant
Spare Parts List and Interchangeability Record (SPIRS).
Red Line Mark-Up : The RLM drawings are produced from AFC drawings and
(RLM) certified as RLM As-Built drawings by the Contractor and QP
Project Discipline Engineers / Construction Supervisors.
Shall : Term used to indicate requirements strictly to be followed in
order to conform to the Standard and from which no
deviation is permitted.
Should : Term used to indicate that among several possibilities one is
recommended as particularly suitable, without mentioning or
excluding others or a certain course of action is preferred but

7.0 DATA MEDIA FOR HANDOVER OF SOFT COPY TO QP

The QP review and handover process of Dossiers is detailed in Section (13, 14).

7.1 Accepted Media Types:

 Hard Copy (See 8.1)


 ISO 9660 Conforming CD-ROMs
 ISO 9660 DVD-ROMs
 USB 2.0/3.0 External Hard Drive (Windows Compatible)

7.2 Data Media Directory Structure for Handover to QP

The complete directory structures (See 7.2.1) along with all applicable contents shall
be contained in one only of the Media types as defined in section (7.1) on final
handover to QP. i.e. the complete deliverable shall not include multiple CD/DVD’s or
External Hard Drives. This means the capacity of the media shall be large enough to
include the complete deliverables.

Zip files for final handover are not allowed. All file names shall be as defined in
Engineering Standard ES.0.07.0014.

7.2.1 Data Media Directory Structure for Project Dossiers Soft Copy (PDF)

The PDF soft copy of Project Dossiers shall be saved in folders with a Directory
structure arrangement as per the Project Dossiers Sections / Sub Section / Sub Sub
Sections as defined throughout the whole of section (12) of this Standard. The folder
structure shall not exceed the 3 level folder structure. See sample directory structure
in Fig 1 below.

Note: All PDF files submitted shall be none secured and without password. All PDF
files shall be searchable enabled.

7.3 Transportation of Data Media Deliverables:

 Labels for packaging CDs, DVDs and Hard Drives (for transit) shall bear the
words ‘OPTICAL RECORDING MATERIAL’ - “HANDLE WITH CARE” The labels
shall indicate:

 Applicable Contract Number/Project Number


 Project Title,
 Deliverable code provided by PTD department

 Packaging used should be adequate to prevent damage to the Media.

8.0 METADATA TEMPLATE – Form ES.0.08.0016

The list of soft copy files delivered, must be listed in Microsoft Excel Metadata form
ES.0.08.0016. This form shall be completed to include all Metadata information
related to all PDF Files handed over on completion of the Project.

8.1 Hard Copy Media for Handover to QP

The Hard Copy Project Dossiers shall be placed only in standard corrugated boxes
suitable for containing 5 A4 binders (see section 10.1). The boxes shall be as
described below:

 Beige colour, heavy duty, minimum of 200lb test acid free corrugated double wall
construction three-ply with die cut handhold cut into each end with removable top.
Boxes shall be interlocking with no staples or tape required for set up with Internal
Dimensions L x H x W of box shall be 400 x 300 x 330 +/- 5mm Tolerance.

 Each Box shall be separately labelled with a barcode label. See APPENDIX
4–Box Label / Barcode numbering standard

Note: Sample boxes are available on request from PTD Helpdesk.

9.0 PROJECT DOSSIERS GENERAL OVERVIEW

The Project Dossier shall comprise of as many physical volumes, as required, to cover
the Sections/Sub Sections.

The Sections of the Project dossier are arranged as topic specific as follows:

Section 3 - Vendor Documentation
Section 4 - Requisitions and Purchase Orders
Section 5 - Design Documents
Section 6 - Environmental Documentation
Section 7 - Project Certification

Section 9 - As-Built Drawings

Considering this arrangement, Contractors engaged in compiling the Dossier shall


note that individual Sections will be compiled as the Data becomes available during
the progress of the Contract.

It shall also be noted that Sub-Contractors may be engaged in different stages of the
Contract and as such may prepare the relevant Data pertaining to any of the above
sections. The main Contractor shall be responsible for retrieving and including all
relevant Data pertaining to the above Sections and Sub-Sections and compiling the
complete Project Dossier in accordance with this Standard.

10.0 PRESENTATION OF PROJECT DOSSIERS

The project data presented in the Dossiers shall be of high quality whether copies or
the original wet signed version. Data that is deemed illegible by QP shall be replaced
as and when requested.

Only the latest approved revisions of Data stated in this standard shall be inserted in
the dossiers. Old revisions and superseded Data shall not be included. Project
administration data inclusive of Memos, Faxes, and Transmittals etc. shall not be
included.

For other Non Applicable Data Specific to the Project refer to (Section 10.3)

10.1 Binders

Each Project Dossier Volume shall be Loose Leaf Bound, in a Hard Cover Binder.

The Binders shall be A4, 4 D Ring, Standard 65 or 70 mm Spine width, with ring
locking mechanism. The Binder shall be of top quality with white hardcover with
transparent pockets on front and spine to hold the dossiers description formats.

Care should be taken not to overfill the Binders. A minimum space of 5mm shall be
kept free in each Binder enabling ease of handling within the Binder and for addition
of extra sheets in the event of modifications/additions.

The Spine and Front Cover of the Binders shall have Transparent Covers to contain
the Title, Section and Volume reference etc., see Section 10.2 below.

10.2 Dossier Description Inserts Format

The layout of the Description Inserts for the cover and spine of the Dossier shall
generally follow the format as shown in Appendices 1 & 2. These are to be inserted
into the transparent front and spine pockets of each volume as appropriate.

The Spine insert shall be colour coded according to the pertinent section as defined
below – refer to APPENDIX 1 for positioning of the colour band.

Section 1 - Dossier User Guide = Red


Section 2 - Overall Plant Operating Instructions = Dark Blue
Section 3 - Vendor Documentation = Purple
Section 4 - Requisitions and Purchase Orders = Brown
Section 5 - Design Documents = Grey

If more than one Section is included in the binder, multiple colour of the Section shall
be shown on the Spine Insert Format.

10.3 Non Applicable Data

The use of abbreviation N/A shall be used in the dossier Indices and Fly sheets to
indicate Section /Sub Section that are not applicable.

If a complete section is not applicable, there is no requirement to list all of its Sub
Sections and Sub Sub Sections in the main Index. For example if all of Section 2 and
3 pertaining to a project is not applicable, it can be shown as in the example below:

Similarly, if a Complete Sub Section is not applicable there is no need to show all of its

If one or more Sub Sub Sections is applicable then all the Sub Sub Section titles shall
be shown. The same principle will apply to the Section and Volume Indices.

For Sections/Sub-Sections that are not applicable, the detail shall be indicated on the
flysheet of the previous Sections/Sub-Section. Dividers and flysheets are not required
for these sections.

10.4 Dividers and Flysheets

10.4.1 Dividers - Sections

Sturdy Cardboard A4 - Numerical Tabbed Dividers shall be used to divide each


Section.

10.4.2 Dividers – Sub-Sections

Sturdy Cardboard Coloured A4- Tabbed Dividers shall be used, to divide the Sub-
Sections up to 3 Subdivisions – i.e.: 1.1 / 1.1.1 and shall have there applicable Sub-
Section or Sub-Sub Section numbers either custom provided or stencilled on each

Tab using permanent ink. Use of stickers is not permitted. Colour A4 sheets shall
separate any further Divisions.

10.4.3 Flysheets

Flysheets shall be inserted at the beginning of each Section and shall list each Sub-
Section title. Flysheets shall also be inserted at the beginning of each Sub-Section
showing the title of the Section followed by the title of the Sub-Section. The Flysheets
shall generally follow the format shown in APPENDIX 3.

10.4.4 Sheet and Page Sizes

All documents/drawings contained within the dossier shall be formatted or folded to A4


size as follows:

 All A3 size sheets to be folded to A4

 Where practicable, drawings larger than A3 shall be reduced to A3, then folded to
A4 size.

This does not include Red-Lined Mark-ups or As-Builts for which the Original
Size Drawing (with wet signature approval) shall be inserted into the dossier
inside transparent sleeve/wallet.

 For drawings where reduction to A3 renders them illegible, the original size
drawing shall be folded to A4 size and inserted into transparent sleeve/wallet.

 Documents smaller than A4 shall be firmly affixed to an A4 sheet.

 All irregular size or shaped documents such as circular data charts are to be
inserted into transparent sleeve/wallet.

 Contractors shall ensure that all Data used in compiling the Dossiers is clearly
legible and all paper used will be suitable for long-term storage (minimum
80gms/m2)

11.0 NUMBERING OF PROJECT DOSSIERS

The Project Dossier shall be numbered uniquely using a QP numbering system (refer
ES.0.07.0014) as explained below:

a) For Greenfield Projects: YYYY-0-40-0001

b) Existing Plants: XXXX-YYYY-0-40-0001

Where XXXX = Project number for Projects used for modifications/upgrades to


existing plants,

YYYY = Applicable Location Code, or for multiple areas to use the applicable plant
General and Whole Plot Areas Location Code,

0 = Discipline Code i.e. General and Multi-Discipline,

40 = Sub Group and


0001 = Sequence Number

The Number shall be placed on the Spine and Front Cover of Each Volume. (Refer to
APPENDIX 1 and 2).

Note: The Project Dossier Number is used for Future Archiving and Data storage and
retrieval after handover of the Dossiers. The same may be applied for Document
Control during the development and review of the Dossiers.

11.1 Individual Volume Numbering

Each volume shall have a unique number and each volume shall be numbered
specific to the section number i.e. Section 1 of the project dossier may contain 3
volumes, so they shall be numbered:

 Section 1 Volume 1
 Section 1 Volume 2
 Section 1 Volume 3

More than one section may be included in a single volume set. As such, this shall be
clearly defined on the front cover and indices. See example below:

 Section 1 Volume 1
 Section 1 Volume 2
 Section 1 and 2 Volume 3

The total amount of Volumes within each section and full set of dossiers are identified
on the respective indices. See APPENDIX 4, 5, 6 and 7.

12.0 PROJECT DOSSIERS CONTENTS

12.1 Section 1: Title - Dossier User Guide

12.1.1 Section 1.1: Dossier Content Guide (Main, Section and Volume Indices)

This Section shall give a brief guide to the contents of the Dossier indicating where
each index can be found. The indices shall be inserted as follows:

Main Index – Section 1.1.1 and front of each Volume. For sample, see (APPENDIX 6)

Section Index – Section 1.1.2, and beginning of each Section and front of each
Volume, the Section spans/extends. For sample, see (APPENDIX 7)

Volume Index – Section 1.1.3, and beginning of each Volume. For sample, see
(APPENDIX 8)

12.1.2 Section 1.2: Project Dossier: Project Synopsis

The Synopsis shall give a general overview of the completed Project. The Synopsis
shall be restricted to 4 or 5 pages maximum. The contents shall include but not limited
to the following:

 Project Title
 QP Key Project Team Members
 Objective/Aim
 Technical Overview/Design Basis
 Benefits to QP
 Difficulties Experienced
 Improvement Suggestions

12.1.3 Section 1.3: Equipment Tag Key List

The Equipment Tag Key Lists shall be prepared in order to provide a quick location
reference in the Project Dossier for Key associated with the applicable Tag Number,
inclusive of Vendor supplied skids and packages.

The Equipment Tag Key list shall be provided in MS Excel format and shall follow the
format as shown in Appendices 8. The MS Excel form ES.0.08.0016 is available on
Engineering Services (PT) Intranet Website and can be found in the “Forms” link.

The hard copy print shall be inserted into this Sub Section on completion of all data
entries. The Native file of the Equipment Tag Key List shall also be issued at handover
stage (See Section 14.0).

12.2 Section 2: Title - Overall Plant Operating Instructions

Section 2 to include relevant Section 2 Index.

12.2.1 Section 2.1: Introduction

The introduction shall outline the Project Scope of Work as completed, detailing all
changes to the original Scope, Project Execution Plan (PEP) subsequent to the
approval of the Basis of Design document and giving reasons for such changes.

12.2.2 Section 2.2: APPENDIX A of Contract

The document as approved by QP complete with a summary list of all approved


Contract Variations and endorsed “Deviation Requests”.

12.2.3 Section 2.3: Plant Operating Instructions

All relevant Documents and Drawings as referenced in the following Sub-Sections


shall be included.

related Data not defined above. Additional Sub Sub-Sections shall continue with the
next available Sub Sub-Section number i.e. 2.3.5

12.3 Section 3: Title - Vendor Documentation

Vendor documentation shall bear the numbering system as defined in Engineering


Standard ES.0.07.0014. If a vendor’s information is in a separate book than the
dossier then this should be stated in the applicable Sub-Sub Section and reference
made to the book number.

Where the Vendor supplies the information and it cannot be compiled into the format
defined in this Section then each Section shall contain a cross-reference to the
Section where the information is contained. Vendor Documentation shall be filed by
Discipline/Subject (see 12.3.1) and then by Indent Number.

12.3.1 Section 3.1: Vendor Drawings, Parts Lists and Data Sheets etc.

Section 3.1 shall include all Vendor Drawings, Parts Lists, and Data Sheets with
cross-reference to the applicable Purchase Order Nos. Sub-Sections of Section 3.1
shall be arranged by Discipline/Subject as shown in APPENDIX 12.

12.3.2 Section 3.2: Spare Parts List and Interchangeability Record (SPIR) Forms

Approved AFC stamped SPIR forms; SAP Coded by QP Materials Department for all
Equipment purchased shall be included in the Project Dossier.

Sub-Sections of Section 3.2 shall be arranged by Discipline/Subject as shown in


APPENDIX 12.

12.3.3 Section 3.3: Manuals and Other Vendor Documentation


Section 3.3 shall include vendor equipment Commissioning, Installation, Operation
and Maintenance Manuals. Sub-Sections of Section 3.3 shall be arranged by
Discipline/Subject as shown in APPENDIX 12.

12.4 Section 4: Title – Requisitions and Purchase Orders


12.4.1 Section 4.1: Requisitions

Section 4.1 shall contain all Approved Purchase Requisitions. The Sub-Sections of

13.0 QP REVIEW AND APPROVAL OF PROJECT DOSSIERS

The QP Project Team and applicable QP Plant Asset owners/operations review (if
required as per Contract Scope) shall be focused on the Technical Contents of the
Project Dossiers, where the QP Engineering Design, Info & Standards Division (PTD)
(as below) review is focused on the Compilation in accordance with this Standard, its
accuracy and completeness as per approved Indexes.
The review for compliance check to this Standard shall be submitted in stages and
format as detailed in the below table:

compiling the hard copy dossiers in accordance with this standard.

14.0 FINAL PROJECT DOSSIERS HANDOVER

Prior to final handover of Project Dossiers Hard Copy Contractor shall contact relevant
QP PTD representative to arrange location and delivery date of hard copy dossiers.
The Final handover shall consist of the following:

A) Soft copy Submission of Project Dossier - PDF files


B) Completed Metadata Template (Form ES.0.08.0016) in MS Excel
C) Equipment Tag Key List (Form ES.0.08.0017) in MS Excel
D) Hard Copy Submission of (the full set of) Project Dossier (Original Wet Signed
Approval as applicable)

On receipt of the above, QP PTD division will conduct final checks. The appropriate
QP Project Engineer or QP Departments shall be notified of any discrepancies in the
deliverables and shall be responsible for obtaining the revised / corrected or missing
data.

The QP PTD division shall inform the QP Project Engineer or QP Department of final
acceptance of all Project Handover deliverables as defined above; It shall be noted
that the project IS NOT complete until QP PTD division approves the complete
deliverables set.

15.0 SECRECY UNDERTAKING

Upon contract completion, all contract documents and Engineering Data


(including data supplied by QP or developed by contractor pursuant to the
contract work scope) must be submitted to QP. Unless contractor requests and
QP approves of retention of documents and data beyond the completion date,
contractor lacks authorization to retain. Accordingly, unless contractor
specifically requests authorization to retain documents and data beyond
completion and QP agrees in writing to such request, it will conclusively be
presumed that all contract documents and related data has been submitted to
QP in accordance with the applicable confidentiality provisions of the GCOC
(General Conditions of Contract).
