LIST OF DOCUMENTS TO BE BROUGHT AT THE TIME OF PHYSICAL

REPORTING IN THE UNIVERSITY:

The candidates at the time of reporting/admission must bring the following original
certificates/documents/testimonials mentioned below category-wise(as applicable)
along with two sets of duly attested/self attested photocopies with them.

Common documents required for all students

1. Offer letter received from JCB UST YMCA, on your registered mail ID.
2. Receipt of Token Fee paid
3. 10+2/ Equivalent exam. Mark sheet( please read the eligibility criteria in chapter-3 of
university Information Brochure to know the required percentage and subjects for the
course you want to take admission)
4. Matriculation/High School Certificates.
5. Character Certificate/ Gap Year affidavit.(If you have passed your 10+2 exam
submit character certificate from principal of school otherwise submit Gap year
affidavit on notary Rs. 10/- stamp paper as per standard format)
6. Medical Fitness Certificate as per annexure XIII of Information Brochure 2019-
20)
7. Two Passport size color photographs
8. Anti ragging affidavit (as per annexure XI & XII of Information Brochure)
9. Aadhar Card copy
10. Remaining fee in form of DD in favour of “Registrar, J.C. Bose, UST Faridabbad)

Additional Documents required for RoHC category candidates

1. Residence/Domicile certificate of any state other than Haryana OR Proof of passing

your 10+2 exam from a school located outside Haryana.
2. Undertaking / Declaration (on Rs. 10/- stamp paper) mentioning following points :- (i) I
am not the resident of Haryana (ii) I have passed my 10+2 exam from a school located
outside Haryana (iii) I am not taking benefit of haryana resident for admission in any
Institution /University

Additional Documents required for HOGC category candidates

1. Haryana domicile certificate (if 10+2/ Equivalent exam. is passed from an institution
situated outside Haryana)
Additional Documents required for SC Category candidates

1. Haryana domicile/Residence certificate issued by competent authority

2. SC Certificate (as per Format attached Annexure-IV in information Brochure)

Additional Documents required for BC-A/BC-B Category candidates

1. Haryana domicile/Residence certificate issued by competent authority

2. BC-A/B certificate (as per Format attached Annexure-V in information Brochure)
3. Affidavits of both mother and father (as per Format attached Annexure-VI in
information Brochure)
4. Annual Gross Family Income certificate from competent authority

Additional Documents required for PH Category candidates

1. Haryana domicile/Residence certificate issued by competent authority

2. PH certificate (as per Format attached Annexure-VII in information Brochure)

Additional Documents required for EWS Category

1. Haryana domicile/Residence certificate issued by competent authority

2. EWS Income and Asset Certificate ( As per annexure XIV of Information
Brochure)
Additional Documents required for TFW Category

1. Haryana domicile/Residence certificate issued by competent authority

2. Income certificate issued by the competent authority( Annual income of
parents/Guardians should be less than Rs. 8.0 Lakh from all sources).

Additional Documents required for HOGC- ESM/FF

1. Haryana domicile/Residence certificate issued by competent authority

2. Ex-service Man Certificate(Annexure- IX/X) or Freedom Fighter Certificate( Annexure-
VIII) as applicable.
Additional Documents required for SC-ESM/FF Category candidates

1. Haryana domicile/Residence certificate issued by competent authority

2. SC Certificate (as per Annexure-IV in information Brochure)
3. Ex-service Man Certificate(as per Annexure- IX/X) or Freedom Fighter Certificate(as per
Annexure-VIII) as applicable.
Additional Documents required for BC-A (ESM/FF)/BC-B (ESM/FF) Category candidates
1. Haryana domicile/Residence certificate issued by competent authority
2. BC-A/B certificate (as per Annexure-V in information Brochure)
3. Affidavits of both mother and father (as per Format attached Annexure-VI in
information Brochure)
4. Annual Gross Family Income certificate from competent authority
