CONTRACT OF LEASE

KNOW ALL MEN BY THESE PRESENT:

This contract of lease entered in the City of Mandaluyong, this ____ day of _________, 2018
by and between:

Miss Thelma F. Correa, of legal age, married/single, with residence at 3399 San Jose St., Sta.
Mesa, Manila, who is duly and sole owner of the Unit 19D rev. Aglipay St. Bellamaja Townhouse
Mandaluyong, hereinafter called the LESSOR.

JERALYN P. ESTACIO of legal age, single/married with resident of Block 38A Marteniz St.
Brgy. Welfare Ville, mandaluyong City employed at Helam Management Corporation at 575 Nueva
De Febrero St. Mandalulong City hereinafter called the LESSEE.

WITNESSETH

1. That for and in consideration of the stipulation hereinafter agreed upon, the
LESSOR hereby demices and leased unto the LESSEE the residential UNIT 19D
Bellamaja Townhouse, 411 Rev. Aglipay St., Mandaluyong City with are belonging to the
LESSOR.

2. That it shall be understood that this Contract is on a month to month basis and at a
rental of five thousand pesos (P 5000 ) payable in advance within the first five (5) days of
every month at the residence of the LESSOR without necessity of demand.

3. That the LESSOR hereby acknowledge receipt of the amount of ten thousand pesos
(P10,000) which shall be considered as follows:

a) P 5000 as advance rental covering the period of June 30 to July 30 , invoice


number.# 5993
b) P 5000 as one month equivalent deposit, fixed non-bearing interest of which deposit
will have to answer for lease and damages, which may be caused by LESSEE, his
children, visitors, or for any cause, due to his carelessness or negligence in the
premises under lease to have such damages, immediately replaced at his or his
expense, at the termination of this contract without any reimbursement of the cost
thereof. Inv. 5992

4. That the LESSEE will not take any alteration or improvements in the premises,
without first securing the written permission or consent of the LESSOR; and any such
improvement or additional introduced in the building or premises shall become the property of
the LESSOR at the termination of the lease contract, without any reimbursement of the cost
thereof.

5. That the LESSEE shall keep the leased premises in good condition at all times by
making the minor repairs and outsides the leased premises.

6. That the LESSEE shall not sub-leased and/or lease the whole or any portion of the
consent of the LESSOR.

7. That the LESSEE shall not conduct any immoral or any illegal business in the
premises, nor shall keep therein pigs, dogs,cats, chickens, horse stable at any household
project which may cause disturbances, or determination to the neighborhood.

8. That the LESSEE shall use the premises as a private dwelling house only; and not for
any purpose neither commercial.
9. To permit the LESSOR-OWNER or his agent or representative to enter and inspect the
conditions Inside the house of the premises at any time.

10. That all water and electrical consumption shall be paid by the LESSEE promptly every
time such are due and to deliver the receipt of payment to the OWNER-LESSOR, as
evidence or compliance.

11. That the premise under lease shall be exclusively used by the LESSEE and members of
the family and one maid. The number of such family to reside therein and only 6. Any
additional person to reside in the said house subject of the contract the LESSEE shall pay
an additional lease in an amount of P1, 000.00 per person.

12. That all ordinary expenses incurred or that may arise in the daily use of the toilet
facilities and sewers in the premise shall be for the account of the lessee.

13. Any delayed in payment of monthly rental as it become due shall automatically bears
interest of 14% per annum aside from the collection expenses incurred for its full
payments.

14. To inform the LESSOR in writings fifteen to thirty days in advance of any intention on
the part of the LESSEE to vacate the premises or prior to such intended vacation.

15. That to secure the prompt payment of obligation due the LESSOR, LESSEE agrees to
put up all its properties brought into the premises as collateral and empower LESSOR to
take possession of the same and sell them to, public auction through any duly authorized
auctioneer to satisfy whatever amount is due and payable to the LESSOR.

16. That the place is with itemize appliances and furnitures as part of this Lease Contract,
that the event appliances and furniture where damages, lost and destroyed it be
repaired, replace with the same quality and paid for by the LESSEE;

17. Any disagreement between the parties, will be earnestly ironed out, and/or settled by them
in a cordial and harmonious manner, however, if they fail to agree or settle such differences
or disagreement they may resort to the aid of the lawyer, in such event, the party found to be
fault, binds himself to pay unto the winning party, a liquidated damages in the amount of
P10, 000.00 to pay attorney’s fee, and the cost of suit. This provision shall likewise apply
against the LESSEE, should he/she fail to comply or violate any or all of the terms and
conditions therein above stipulated.

18. Failure of the part of the LESSEE, to comply with any or all of the forgoing terms and
conditions as
are therein stipulated, will be sufficient cause, to terminate this Contract.

known and known to me to be persons who executed the forgoing instruments, and acknowledge to
me that they executed the same as their free act and deed.

This instrument consisting of three (3) pages, including this where the acknowledgement is
written, has been signed by the party and witnesses on the left margin of each pages and every pages
thereof and sealed with my notarial seal.
