It has come to my knowledge that the position for Nurse II- Operating Room Nurse is now open
for application.

It is in my great interest and enthusiasm to apply for the position of Nurse II- Operating Room
Nurse at Davao del Sur Provincial Hospital. I am VILMA A. ESPERANZA, RN 59 years of
age and of good health and presently residing at Barangay Aplaya, Digos City. I am a graduate
of University of Bohol with a degree in Bachelor of Science in Nursing, batch 1980. I have taken
and passed the Board Examination for Nurses in 1981.

I am recently connected with Davao del Sur Provincial Hospital as a Nurse I Operating Room
Nurse. My service in the hospital has reached 30 years now. I was previously assigned in the
hospital ward from 1986 to 1990 and the Emergency Room from 1991 to 2000. In the year 2000,
I was re-assigned in the Operating Room due to lack of manpower. I have created a great level
competency in my job as an Operating Room Nurse for 16 years. I feel very confident that my
sufficient training and educational attainment aside from my strong determination, dedication
and commitment to excellence guarantee that I can meet the requirement of an effective Nurse
II- Operating Room Nurse.

Core Functions:

1. Assists the Surgeon on Major and Minor Surgical Procedures

2. Circulates on Major and Minor Surgical Procedures

3. Performs accurate charting.

4. Monitor patients in the Post Anesthesia Care Unit (PACU). Carries out Doctors’ Orders

5. Supervises nursing attendants and Job Order Nurses

Support Functions:

1. Prepares complete instruments according to surgical operation.

2. Transports and endorses patients to nurses assigned in the ward

3. Make Daily Time Record (DTR), MPOR and IPCR

4. Prepares supplies needed for OR Use

5. Attend meetings and seminars

6. Maintains cleanliness in the OR
