a) Manipulación gingival
1- Válvulas protésicas cardiacas, incluyendo prótesis valvulares por vía percutánea (TAVI) (3,4)
2- Material protésico empleado para reparación valvular como: Anillos (anuloplastía) y cuerdas.

5- Trasplante cardíaco que presente regurgitación valvular, por daño estructural valvular (7)

Las decisiones sobre el momento de la intervención quirúrgica deben ser efectuadas por un
equipo multidisciplinario en valvulopatías y que incluya, Cardiocirujanos e Infectólogos (255).

El riesgo de daño neurológico post operatorio es bajo, después de eventos neurológicos que
no hayan significado daño extenso parenquimatoso o hemorragia intracraneal.
Si la cardiocirugía es necesaria luego de un evento neurológico, bibliografía reciente favorece
la cirugía temprana, para la obtención de un mejor desenlace clínico (284,285).
Diferir la cirugía valvular por al menos 4 semanas, puede considerarse en pacientes con EI y
un Ictus isquémico extenso o una hemorragia intracraneana, si el paciente está
hemodinámicamente estable. (286)
Comentario:
En esta edición 2017 de estas guías que aluden a Endocarditis Infecciosa, no se aprecian cambios
importantes, salvo la precisión respecto al peligro de efectuar cirugía valvular, antes de las primeras
3-4 semanas de producido un Accidente Vascular Encefálico; sea por infarto extenso o hemorragia
intracraneana, lo que produciría elevada mortalidad (286).
