AFFIDAVIT OF ARRESTING OFFICER

I, Pat Michael C Destura ,Intel Operative of Mabitac MPS , a bonafide member of the
Philippine National Police (PNP) presently assigned at Mabitac Municipal Police Station
with office address at Brgy Maligaya Mabitac, Laguna, after having been duly sworn to in
accordance with the law do hereby depose and state:

That at about 09:30in theevening of June 3, 2019, DET/Intel personnel of this station led bySPO2
Randy R Lopez with five (5) others under the direct supervision of PLT ROMEO S MANGA, OIC
conducted Anti-illegal Drug (Buy-Bust) Operation in consideration of Five-hundred-peso bill as
marked money bearing serial No. 6094990under PDEA4A Control No. 10005-062019-0097along
the Provincial Road ofBrgySinagtala, Mabitac, Laguna that resulted in the arrest of the above-
named suspectafter Selling/ peddling One (1) piece of small heat-sealed transparent plastic
sachet containing white crystalline substance suspected shabu with marking MCD-BB to the
Police Poseur Buyer. Upon apprehension, suspectwas subjected to preventive search which
eventually resulted in the confiscation of two (2) pieces of small heat-sealed transparent plastic
sachet containing white crystalline substance suspected shabu with marking MCD-1& MCD-2and
recovery of the used marked money and two (2) pieces of one-hundred peso bills, One (1) pc of
twenty peso bill, One (1) pc of five peso-coin and Nine (9) pcs of one-peso coin suspected drug
money with a grand total of Php 234.00. Thereafter, suspect was apprised of his constitutional
rights and brought to this station for documentation and proper disposition.Furthermore, Suspect
and the confiscated drug evidence will be brought at Provincial Crime Laboratory, Sta Cruz,
Laguna for chemistry and Drug Test examination.

The actual physical inventory, markings and taking of photographs were conducted
at the crime scene and witnessed by the suspect himself in the presence of Brgy
Chaiman PAOLO VILLANUEVA , Kag. BERNARD CAPACETE, Kag.JOHN MARK PAGOLA and
Mr. De Roma, Media-GMA 7.

That i brought the arrested suspect and all confiscated pieces of physical evidence
to our office for investigation and proper disposition. The arrested person was latter
brought to General Juan Cailles Memorial District Hospital, Pakil, Laguna for physical
examination and further brought to PNP Laguna Provincial Crime Laboratory for Drug
Testing and the confiscated pieces of physical evidence were turn-over by the
undersigned together with the duty investigator to Laguna Crime Laboratory, Sta Cruz,
Laguna for chemical analysis and it was received by Pat Solano at about 07:00 PM of
April 24, 2019;

That the arrested person was latter identified as PAUL JOHN PASCO y
FERNANDEZ @ JHONG, 26 years old, married, driver, born on June 18, 1992, a resident
of BrgySinagtala, Mabitac, Laguna.

That anything not mentioned in this affidavit will be narrated during proper trial;

That i executed this affidavit to attest to the truthfulness of the above-narrated

facts and to support our charge for Violation of Sec Sec 5 & 11 RA 9165against the
above-named suspect.

hereby depose and state:

That, we are presently assignd at Mabitac Municipal Police Station as investigators;

That at about 09:30in theevening of June 3, 2019, DET/Intel personnel of this station led

this station. That the undersigned was designated as investigators-on-case of the said operation
and have caused the preparation of the following documents:

That the pieces of drug related evidence recovered by the Seizing Officer (Pat Destura )
was presented (Only shown) to duty Desk Officer for documentation;

That at about 07:00 pm of June 3, 2019, the arresting officer properly turned-over the
pieces of drug related evidence duly recieved by Pat Solano..

That the arrested suspect was also presented by the arresting officer to the undersigned
for investigation. Suspect was brought to PNP Laguna Provincial Crime Laboratory for Drug
Testing and to Pakil Hospital for Physical Examination;

That, we executing this affidavit to attest to the truthfullness of the foregoing statements
and to support the filing of appropriate charges against the above-named suspect.
