1. It is hereby a matter of record that SONNY V. MATIAS, has been designated as

School SPG Adviser effective June 22, 2018.

2. As such, he is tasked with the following duties and responsibilities.

1. Acknowledge receipt of advisories and reminders from the Principal.

2. Represent/spearhead the school SPG activities.
3. Ensure the availability of baseline data of the group being represented;
4. Submit situation/narrative reports and provide real time updates to the Principal;
5. Does other related works.

Unless previously authorized, there shall be no deviation from the foregoing duties
and responsibilities and that any violation thereof, shall subject him/her to disciplinary
actions.

This designation is only temporary in nature and in the interest of the service may be
recalled anytime by the undersigned subject to existing rules and regulations.

DESIGNATION

It is hereby a matter of record that SONNY V. MATIAS, has been designated as District
Science Coordinator effective 2013-2014 and onwards.

As such, he is tasked to perform with the following duties and responsibilities.

1. Leads in the construction of tests in science;

2. Coordinates all activities, programs and projects in its smooth conduct and
implementation;
3. Prepares district action plan in science for approval and implementation;
4. Consolidates and interprets assessment results and recommends intervention
program to address least mastered skills; and
5. Prepares training matrix in science for approval and implementation.

Unless previously authorized, there shall be no deviation from the foregoing duties
and responsibilities and that any violation thereof, shall subject him/her to disciplinary
actions.

This designation is only temporary in nature and in the interest of the service may be
recalled anytime by the undersigned subject to existing rules and regulations.
