Reason for Referral: The original referral to Help Me Grow (HMG) was made by DT, Mother
to make sure he develops on track. The referral was made in 2017 due to a congenital heart
defect. The assessment results in this report were done for a school project focusing on IFSP
creation, implementation and progress monitoring.

Background information: K was born at 39 weeks. At 24 weeks gestation, he was diagnosed


with Hypopolastic left heart syndrome (HLHS). 4 days after birth, he had surgery to place a Sano
shunt. He was in the NICU for 4 weeks and discharged in October. He has follow-ups with a
cardiologist to monitor his heart condition. August he will be going through tongue-tie repair
surgery. He lives with his mother, D, and his 13-year-old brother. His father is intermittently in
his life but not considered a strong resource, according to mom. Family supports include K’s 3
aunts (maternal side), 5 cousins, HMG, and Akron Children hospital. Mom reports that she
works closely with the social workers at the hospital. They are a strong support system for mom
and K.
Current Assessment: The Communication IGDI and Communication portion of the AEPS
were done to assess K’s communication skills. Mom’s made concerns were communication so
both assessments were done. The IGDI video showed that the child has vocalizations and
gestures as forms of communication. The AEPS showed that the child does consistently use
word approximations, does back and forth vocal exchanges, can follow simple one-step
instruction and sometimes two steps, with contextual cues. He does not use 2-3-word phrases
yet, but he is not even two so that is not an area of concern. He will follow gazes and look when
someone points at an object. He has names for familiar people and can label a small number of
familiar objects. Mom reported that he does not consistently point to objects he wants, he will
not consistently gain a person’s attention and point to an object, and he does not have gestures or
vocalizations to reject or protest objects or actions.

Recommendations: Based on the assessment results, and his medical report of a tongue tie,
working on prelinguistic communication skills would be most beneficial. I will introduce
strategies to increase pointing, appropriately getting someone’s attention when he needs his
needs met, and having vocalizations and gestures for no. He has positive skills gestures and
vocalization, but not for functional use. Due to the tongue tie, his word use may not increase but
we can focus on foundational communication skills and hope his word communication will
increase after the tongue tie revision.
