Dear Doctor,

Greetings in the name of Public Service!

The Baguio City Jail Female Dorm is scheduled to conduct its Food Safety Orientation on
April 4, 2019 at 2’oclock in the afternoon. This is to accommodate those who are due to
renew their Health/Sanitary Permit and issue the same to those who have not availed of
it yet but are required to get one.

It is in this connection that we wish to invite a representative from your HSO – Sanitation
Division to hold the necessary seminar as a pre-requisite to the issuance of the Health
Permit. In addition, please be informed that most of the activities in this facility involve
food handling; thus, the need for the orientation. This will also be the opportune time for
your Sanitation Team to evaluate our kitchen facilities if it remains to be compliant with
sanitation standards.

Should you have queries, please text or call JO2 Canalyn Alhambra, Duty Nurse, at cellular
phone number 09267674262.

Thank you and hoping for you kind indulgence on this matter!

Respectfully yours,

APRIL ROSE W AYANGWA

Dear Doctor,

Greetings in the name of public service!

The Baguio City Jail- Female Dormitory with its dedication to public service would
like to extend its program on the observance of the Oral Health Month. However this
facility has no capability to address the major health concern of Person Deprived of
Liberty (PDL) nowadays, as to their common oral health problem.

In view thereof, may we respectfully requests your good office to conduct tooth
extraction, dental cleaning/oral prophylaxis and lecture to promote good oral health
practices to attain the saying “Ngiping Inaalagaan at Pinagyaman, Hatid ay Ngiting di
