VIDEO PRODUCTION CONSENT FORM

Without expectation of compensation or other remuneration, now or in the future,

I hereby give my consent to Collins International Trading Corporation, its affiliates
and home brand (Dowell), to use my recipe videos in its publications, advertising or
other media activities (including social media). This consent includes, but is not limited
to:

(a) Permission to use my recipe videos;

(b) Permission to use my brand logo; and

(c) Permission to use in any forms of publications, advertising and media

activities (especially social media) and/or in mailings for promotion and
awareness.

I hereby waive any right to inspect or approve video materials, photographs, advertising
copy and/or printed matter that may be used in conjunction herewith, or to any eventual
use to which it may be applied. In the event that Collins should alter or modify my
recipe videos, I will not hold Collins liable therefor.

This agreement constitutes the sole, complete and exclusive agreement between
Collins International Trading Corporation (Dowell) and myself. I am not relying on any
other representations, whether oral or written, in executing this Release and Consent
Agreement.
