CONTRACT OF SERVICE

This CONTRACT made and entered into this 1st day of JUNE 2019, at Barangay Pinyahan,
District IV, Quezon City, Philippines by and between:

BARANGAY PINYAHAN QUEZON CITY, METRO MANILA, a government entity organized and
existing under and by virtue of laws of the republic of the Philippines with office address at No. 35
Malakas Street, Barangay Pinyahan, Quezon City and herein represented by HON. JESUS P. LIPNICA,
III., in his capacity as Punong Barangay herein referred to as the FIRST PARTY;

- and –

, EVELYN ROCELA, of legal age, and a resident of 102 Mapang-akit Street, Barangay
Pinyahan, Quezon City and herein referred to as the SECOND PARTY:

WITNESSETH –

WHEREAS, Barangay Pinyahan, Quezon City, thru the Gender and Development (GAD) needs
the conceptualization, initiation, planning, and monitoring of the children of the barangay and
formulate activities specially for the effective and efficient delivery of the basic services for the
benefit and welfare of children of Barangay Pinyahan;

WHEREAS, to achieve the foregoing purposes, the services of the GAD Monitoring Team has
been duly appropriated by the Council of Barangay Pinyahan, Quezon City;

WHEREAS, the SECONDARY PARTY, possesses the appropriate qualification, training,

experience, and skills in the effective and efficient planning and implementation of the needed
special projects in Barangay Pinyahan and is willing, ready and able to discharge the said function;

WHEREAS, the effectively of this Contract shall commence on June 1 2019 to December 31,
2019 unless sooner revoked, rescinded or terminated by the FIRSTY PARTY for any lawful reason and
maybe renewed at the sold option of the FIRST PARTY;

NOW, THEREFORE, for and in consideration of the herein mutual covenants and undertakings
herein below provided, the parties here to have agreed as follows:

1. THAT the services of the SECONDARY PARTY, in the person of EVELYN ROCELA – GAD Staff
with a monthly honoraria of Four Thousand Five Hundred (4,500.00) PESOS each effective
June 1, 2019 until December 31, 2019, unless terminated or renewed as above provided;

2. THAT, the FIRST PARTY, shall provide the office space, office equipment and office supplies
needed for the effective and efficient functioning of GAD Development Office of this
Barangay;

3. THAT, the SECOND PARTY shall strictly follow the terms and conditions of this agreement
such as:

a. the SECOND PARTY shall ensure the effective implementation of the GAD Development
and Monitor the welfare of the barangay;
b. the SECOND PARTY shall perform all other duties as required by law and all legal orders
as directed by their superiors.
 4. That, the schedule time of work by the SECOND PARTY shall be from 8:00 a.m. to 5:00 p.m.
daily from Monday to Friday;

5. That in the event of successive holidays only one (1) day could be used as an Office Holiday
with pay and the rest should be a working holiday;

6. That the FIRST PARTY or his representative shall cause the preparation of the Monthly Payroll
and the necessary Disbursement Vouchers to effect collection of their monthly salaries upon
approval of authorities concerned.

IN WITNESS WHEREOF, we affixed our signature this 1st day of January 2018, herein Quezon City,
Metro Manila.
