SUBJECT: PROJECT EMPLOYMENT

This confirms our offer to employ you as Technician for SLI-Innove in connection with our
Third Party Agency (TPA) with Innove Communications, Inc. with the above status under the
following terms and conditions:

1. Your compensation shall be P375.00 per month.

2. You will report directly to Mr. Feliciano E. Felicilda Jr.
3. Your project employment shall commence on April 10, 2017 and end on October 10,
2017. This may be extended until completion of the SLI Project for Innove
Communications, Inc. and coterminous with this same undertaking. It shall cease
thereafter without necessity of notice or separation pay.
4. The Company reserves the right to terminate your project employment even before the
expiration of this contract for any of the following grounds:
4.1 Completion of the work or job or any phase of the Project to which you are
assigned;
4.2 For just causes provided by law:
4.3 When your services are no longer needed as a result of amendments or changes
in Frame Agreement for Voice and Data Subscriber Line Installation
Services Project.
4.4 When performance is below the required standard of the company based on the
two(2) consecutive quarterly Employee Performance Evaluation(EPE).

5. Your employment benefits shall consist of those granted under this Contract, the Labor
Code and related legislation.
6. You shall be entitled to the following company benefits:
 13th month pay to be computed pro-rata
 Philhealth
 SSS
 Pag-ibig fund

 7. You shall have six (6) working days, the work schedule is to be determined by
management.
8. You shall not disclose any information relating to the business, operations and dealings
of the Company, which may or may not be relevant to your assigned work, to any
person or entity.
9. You are expected to give priority to your work schedule especially in regard to rendering
overtime work when needed or required.
10. In case you decide to resign from the Company, you are required to give thirty(30)day
advance written notice. Any or all liabilities you have incurred from the Company
shall be immediately due and demandable.
11. You shall faithfully comply with Company rules and regulations particularly with respect
to working hours, conduct at work, and your relationship with your fellow
employees and the Company’s business clients, associates, and customers.
12. Your absence for two(2) consecutive days without prior permission or justifiable reason
is a ground for disciplinary action.
