and/or your representative to the 1-Day DOLE Prescribed OSH Training Course for Safety Officer 1 (SO1) on September
14, 2019, which will be held at the ISAT-U HRT CENTER LAPAZ, ILOILO CITY. This is a 1-day live out training with a
registration fee of Php2,000.00 per participant that will cover the meals (snacks and lunch), honorarium of speakers, training
kits/supplies and other operational expenses.

The above course is a mandatory training for establishments in the ALL INDUSTRY with the need for a SAFETY OFFICER 1
(SO1) in pursuance to Section 3(v) of the DOLE Department Order No. 198-18, otherwise known as the Implementing Rules
and Regulations of the Republic Act 11058 “AN ACT STRENGTHENING COMPLIANCE WITH OCCUPATIONAL
SAFETYAND HEALTH STANDARDS AND PROVIDING PENALTIES FOR VIOLATIONS THEREOF.”

Furthermore, this course includes learning sessions that will develop/enhance the knowledge, skills, and attitude of the
participants on the basic concepts and principles of OSH towards an effective OSH implementation in the workplace. In
addition, they will be able to know how to respond in workplace emergencies and participate during conduct of emergency drills.
Also, they will gain the confidence as effective trainers in delivering OSH topics for the workers, among others.

This training is highly recommended for OSH advocates, safety officers, OSH committee members, HR managers, facility
engineers/supervisors, company owners/managers, and individuals with relevant fields of undertakings from private companies
and government institutions, the academe, and health care service enterprises.

Cancelation & Refund

Cancellations - by letter, fax, or email only - received by the Secretariat on or before September 06, 2019, the registration fee will be
refunded, less 50% if after the latter date and on or before September 07, 2019 and no refund will be given for cancellations received thereafter,
or for participants who do not attend or partially attend. The COSHEMAP, INC. reserves the right to cancel the seminar in the event of exceptional
circumstances, in which case the registration fee will be fully refunded*. (*All bank charges are to be borne by the participant.)
