AFFIDAVIT OF UNDERTAKING

I, MARIFEH G. PACLIBAR,, of legal age, a Filipino citizen, married, having

a residence at Mandaue City, Cebu, Philippines, after having been duly sworn
to in accordance with law, do hereby depose and state: THAT –

I, am the operation in charge-visayas of V STAR TRUCKING and LOGIS-

TICS CORP here in Cebu. I am authorized to transact, process, sign and move
for the release of our FUSO TANKER with Plate Number MFM 538 under Car
Registration No. 152947001, by virtue of the Special Power of Attorney.

The said FUSO Tanker was unfortunately involved in an incident last No-
vember 30, 2018 and is still in the custody of the arresting officers in Mandaue
City Police Station 3, Mandaue City.

I have caused the preparation and have signed the Urgent Motion to Re-
lease the said Tanker filed before this Honrobale Court.

That, I expressly undertake and obligate myself and V STAR TRUCKING

to have the custody of the said tanker upon its release and shall likewise
undertake to commit our full cooperation in the instant case. We hereby
undertake to obey and follow the Order of this Honorable especially when the
latter orders the presentation of the Truck. We pose ourselves to be
accountable and liable in the event of our failure to abide by the order of this
Honorable Court.

All these acts are being hereby agreed to by me as tantamount to my vol-

untary will and admission of the said incident.

This Affidavit is executed to attest and certify the truth of the foregoing
statement and for whatever legal purpose or purposes it may serve under the
premises.
