1. Submission of a duly accomplished and signed Agreement for the Sa]e of Electric Energy {ASEE]under the General Power
IGP) rate schedule with a contracted capacity of 97.6 kW and a guaranteed minimum billing demand Qf 68.32 kW with a
bermof one{l) year.

2. Payment of bill deposit amounting to Two Hundred four Thousand Eighty and aO/100 Pesos {Php 204,780.00)
equivalent to your estimated ane {l) month billing which shall be correspondinglyincreased/decreasedannually to
approximateYour monthly bill. Said deposit shall earn an interest at the rate prexribed by the Energy Regulatory
CommissionMERC) and shall be refunded upon termination of serflce, provided that all bills have been paid or upon
eligibility for early bill deposit refund;

3. Submission of a duly accomplished and signed Agreementfor Extension of Lines and/or Additional Facilities

4- Payment of CashAdvance amounting to Four Hundred Forty Two Thousand Four Hundred Twenty One and 97/100
Pesos(Php 442,421.97) representing the costs related to the design, installation of facilities and equipment necessary
for the delivery of power to your location

5. installation of exposed service entrance conduits far self40ntained meters regardlessof size of conduaors, except for
those requiring the use of instrument transformers fbr metering purposes; and

6- Ensuringaccessibilityof all Meralco facilities for inspection, reading, and maintenance purposes at all time

Pleasebe reminded that the availability of service and CashAdvance amount will depend on the following

a} The pole location approval from the government agencywith jurisdiction over the roads where Meralco will instal
ts poles;and/or

b} Grant of right-of way from the owner/s af the property that will be affected by the installation of distribution lines
andfacilttiu.
