AFFIDAVIT

I, LETECIA V. MACARANAS, Filipino, of legal age, widow, with residence

address at Prk. 3 Sta. Maria, Nabunturan, Compostela Valley, after having been
subscribed to an oath, depose and say THAT:

1. I am one of the sisters of my late brother, NELSON VICENTE SR.

who died on January 1, 2010 due to ____________________;
2. Before his demise he left his two children, NELSON VICENTE JR
________________ and AYA MONICA VICENTE _________________
under my custody and supervision;
3. My brother was an SSS pensioner and made his children the sole
beneficiary;
4. As their Aunt and guardian, I am their authorized person to
withdraw their monthly pension;
5. I am willing to help and provide what is best for them, but as a
single mother with four children, my income as a Utility Worker will
not be sufficient especially, Nelson Vicente Jr. is now in college;
6. With consent of my nephew and niece, NELSON VICENTER JR and
AYA MONICA VICENTE, they want me to loan their monthly
pension;
7. The proceeds of which should be used for their education and
other daily needs;
