PROJECT SITE SAFETY AND HEALTH POLICY

The Project Safety and Health Policy are to protect every worker from the dangers of injury, sickness or
death through safe and healthy working condition. The purpose is to conduct every work safety and health by
continuous information dissemination.

The management commitment to the implementation of the BOSH program at the project site will be firm
and aggressive so that all work activities are performed with the utmost regard for the safety/; and health of the
workers and the other persons.

All construction workers from top to bottom will adhere to Hazard Reporting Program and must be
responsible in the promulgation of safety and health program and any violation committed will have
corresponding penalty/sanction.

The appointed member of the project Safety and Health Committee will “review, at least monthly, during
the duration of the project, this written construction safety and health policy and maintain and continuously
improve developed OSH Program to ensure that it will remain relevant and appropriate to the health and safety
needs of the construction workers.”

Also, during the implementation of the project, we shall abide and strictly enforce the rules and
regulations of Occupational Safety and Health Standards, As Amended and DOLE D.O. No. 13 Guidelines Governing
Occupational Safety and Health in the Construction Industry.

It is also policy to comply and implement with high standards the requirements of the following:

 National Building Code


 Philippine Electrical Code
 National Plumbing Code
 Sanitation Code of the Philippines
 The New Fire Code of the Philippines

SECTION 1. PROJECT DESCRIPTION

SECTION 2. PROPOSED COMPOSITION OF SAFETY AND HEALTH COMMITTEE AT THE CONSTRUCTION SITE

2.1 SAFETY COMMITTEE will be composed of the chairman, safety man, first aider and 3 markers. Meetings will be
conducted every 1" Tuesday of the month.

Note: Committee Members will be appointed before the start of the project.

2.2 OBJECTIVE: To ensure that Construction Safety and Health Program will be fully implemented at the project
site.

2.3 SCOPE: During construction of the Project only.

SECTION 3. SITE SAFETY AND HEALTH PERSONNEL

The Project Site Safety and Health Personnel will be appointed before the start of the
construction.
3.1 SAFETY OFFICER

SECTION 4. ACCIDENT AND INCIDENT INVESTIGATION AND REPORTING

4.1 ACCIDENTS AND INCIDENTS MUST BE REPORTED. AII occurrences that Is unplanned, uncontrolled and
in some way undesirable which disrupt the normal function of a person or persons that resulted to injury or
near injury will be reported.
4.2 THE FOREMAN IS THE PERSON RESPONSIBLE FOR ACCIDENT OR INCIDENT
REPORTING AND INVESTIGATION. The supervisor is the person who must
investigate and makes out report.
4.3 MINIMUM CONTENTS OF ACCIDENT OR INCIDENT REPORTS
 The exact location, time, vehicles/equipment and people involve in the accident
or incident.
 Take the physical picture of the accident/incident.
 Look for witnesses and interview about the facts.
 Get the facts and accounts from the people involve.

4.4 PROVISIONS FOR RECORD KEEPING OF ACCIDENTS AND INCIDENTS


 The Project Site will keep an accident or illnesses record book which shall be open at all times for
inspection to authorized personnel containing the following minimum data and shall be
maintained by our safety officer.
a. Date of accident or illnesses;
b. Name of injured or ill employee, sex, age;
c. Occupation of injured or ill employee at the time of accident or illnesses;
d. Assigned causes of accident or illnesses;
e. Period of disability ( actual and/or charged);
f. Whether accidents involve damaged to materials; equipment or machinery, kind and
extent of damage, including estimated or actual cost; and
g. Record of initial notice and/or report to the Regional Office or authorize representative.
SECTION 5. GENERAL SAFETY WITHIN THE PREMISES OR AT THE CONSTRUCTION SITE

The Project Owner will install danger signs and safety instructions for workers and Public.
Providing also Construction Safety Signage's to warn the workers and the public regarding the existing
hazards in the workplace. Signages will be posted in prominent and strategic position in English and
Tagalog languages.

5.1 The signage’s that will be installed are but not limited to the following:

1. Mandatory requirement on the usage of personal protective equipment prior to the entry on the
project site.
2. Warning or danger signs on areas where there are potential risks of failing and falling objects.
3. Tripping or slipper areas.
4. Providing electrical danger signs on areas where there are possible contact with electrical
current.
5.2 Project Owner will ensure good housekeeping will be fully implemented in the worksite to prevent
accidents and illnesses.
Construction Site procedures for worksite good housekeeping:
1. Remove all protruding nails or bend them over.
2. Remove all slippery objects on the worksite.
3. Place all work materials in its proper places.
4. Keep loose materials on stairs, walkways, ramps and platforms.

SECTION 6. PROVISION AND USE OF PERSONAL PROTECTIVE EQUIPMENTS.


6.1 The Project Owner will provide each worker with personal protective equipment free of charge
whenever necessary by reason of hazardous work process or environment.
6.1.2 Personal Protective Equipment (PPE) shall be in accordance with Rule 1080 of the Occupational
Safety and Health Standards as amended.
6.1.3 All PPE shall be of approved design and construction appropriate or the exposure at the work to
be performed.
6.2 Mandatory PPE of each worker upon entering the worksite
 Safety Helmets
 Safety Shoes
 Safety Belt (on all elevated work areas)
SECTION 7. FIRST AID AND HEALTH CARE MEDICINES, EQUIPMENT AND FACILITIES
7.1 Authorized personnel will be tap to provide facilities and medicines for the duration of the
proposed construction.
7.2 The Project Owner will fully implement the requirements stated in Do. No. 1 3.
7.3 Available First Aid medicines, Supplies and Equipment is to be provided at the project site.
SECTION 8. EMERGENCY AND DISASTER PREPAREDNESS CONTINGENCY PLAN
The Project Owner ensures that when it comes to any emergency, the workers will be secured and
prepared. The Project Site will implement this Fire Emergency Contingency Plan.
8.1 Fire Emergency Prevention Program
The Project Owner developed this Fire Emergency Contingency Plan that encompasses the
following programs:
8.1.1 Fire Emergency Prevention Program
Project Owner will prepare a program to prevent tire accidents and emergencies from occurring.
Procedures :
 Fire Prevention- Implementation of Good Housekeeping Practices.
 Safety and Health Review- Review Compliance with Occupational Safety and Health Standards
 Inspection- Conducts Workplace inspection to locate tire hazards.
8.1.2 Fire Emergency Preparedness Program
Project Owner will prepare a program that encompasses all activities that are necessary
to prepare the workers to respond to emergencies.
Procedures:
Construction workers with be provided with the following Trainings:
a. Fire Safety
b. Fire Extinguishers Use
c. Electrical Safety d.
d. Good House Keeping

8.1.3 Fire Emergency Response Program


20.2.3.1 Project Owner will prepare program that involves life saving and protection
activities that are implemented during an emergency.
Procedures:
1. Detection, Alert and Warning
2. Direction and Control
3. Communications
4. Emergency Shutdown Procedures
5. Facility Evacuation
8.1.4 Fire Emergency Recovery Program
8.1.4.1 Project Owner will prepare a program that is necessary to continue the Project
Procedures:
 Incident Investigation
 Cleanup and Restoration

8.2 FIRE EMERGENCY CONTINGENCY PLAN DISTRIBUTION

The Project Owner will disseminate this contingency plan to all construction workers.
