1. Terms & Conditions .

For Chili’s Survey Restaurant, there will a contest held for Malaysian
People. Who will be participate individually. Kindly read the below terms:

 Participants Period = It’ll be conducted in January’ period (Malaysian

Festivals)

 Participants Eligibility = a. The candidate should be 18 yrs & above. Its

open for all individual Malaysian people to participate. The candidate
should be vary from Chili’s Survey restaurant.

b. Joining will be instant as per candidates ability.

 The candidates must have receipt copy Chili’s Survey.

a. They can visit the website www.chilissurvey.com or Facebook page with

Chili’s Survey Malaysia to fill up the form given. The form will be Survey
Code, code will be given on each form which is one time used.

b. The candidate should fill up their details truly & completely as per
mentioned and even attached their original documents. The incomplete
form will be disqualified.

 Prizes & Lucky Draw

a. There will one lucky draw winner chosen after every last week of
month in a the Head Office Chili’s.
b. The winner will be given Rs. 5000/- cheque & a gift voucher.
c. The will be informed via email, telephone & letter as per the
form will be filled.
d. The prizes will be not transferable or exchangeable to second
person.
e. The Participant should come to collect their prize to the Head
Office of Chili’s Survey – Tingkat 6, 45E Bangsaria, Jalaan MaarOf,
59100 Kaula Lumpur.
f. The participants should come along with their proof like
Passport/NRCI, to give the particular prize.
g. The prize will be remain unclaimed for 3 months from the Lucky
Draw date.
  General – Participants is deemed to accepted & agreed to the Terms &
Conditions of Chili’s Survey.
Chili’s will be not responsible for any kind damage or lose of the
participant. In fact participants should bear the losses.

Chili’s reserve right to withdraw the participants from promotion if he/she

is not proceeding as per the Chili’s Terms & Conditions.

In event any brochure, marketing/ promotional materials related to the

Promotion the Terms & Conditions will be inconsistence or discrepancies.

These Terms & Conditions are governed by the Law of Malaysia.

 Privacy Policy – The participants who are filling up the form through
Facebook Page the details will be confidential with Chili’s.

 Service Quality – The people who have been visited the Chili’s Survey, give
us the feedback for the Service, Food, Environment.

 Guest Form -- Those who have any queries, complaints, services, etc..
kindly fill the form in given website of on Facebook Page. As per it, if Chili’s
think to change their policies and conditions. If the reasons are truly valid
then Chili’s will make to sure to change it.

Conclusion – Chili’s Survey is a Restaurant which is located in Malaysia. It is governed by

Malaysian Government. They have their social media sites like Face Book & their
particular site. Every year they held one contest with one winner.
And they mention the Terms & Conditions for Participant, who can participate
follow their rules & regulations.
