AFFIDAVIT OF WITNESS

I, SHIRLEY RECTO, of legal age, Filipino, and with postal address at 256 Sto.
Rosario St., Angeles City Pamapanga, after having been duly sworn in accordance with
law, hereby state:

1. That I am the daughter of FAUSTO RECTO with postal address of 256 Sto.
Rosario St., Angeles City Pamapanga;

2. That Mr. MICHAEL CRUZ has rented a house from Mr. FAUSTO RECTO by
virtue of a Rental Agreement dated July 13, 2019;

3. That he resided on the rented house from July to Month of October.

4. That, as stipulated in the Rental Agreement, MICHAEL CRUZ has agreed to

pay the amount of the monthly rental of 6,249.58 Pesos.

5. That after MICHAEL CRUZ paid the first month (July) he failed to pay the
remaining amounts of his time of residence from August to October.

6. That the foregoing act is in direct violation of the aforementioned Rental

Agreement.

7. That pursuant to the Rental Agreement, he is liable to pay in damages Twenty

Five Thousand Nine Hundred Eighty Seven Pesos And Fifty Six Centavos (PhP
25, 987.56).

8. That I attest to the truth of all the foregoing for all legal intents and purposes.
