Agreement
This agreement, between Angel Zamora (Buyer) having an address of 144 caribe ct , greenacres, FL 33413 and American
Musical Supply, Inc. (Seller), is made on 8/7/19 and is with respect to the sale of the following items:

Agreement to Pay In Installments

Buyer agrees with Seller that as an alternative to paying in full at the time of Buyer’s order of the items described above,
Buyer has applied for the ability to make payments under an installment plan according to the following terms of this agreement. The
schedule of payments is as follows:

* Tax, Shipping, and a one-time processing fee of $14.95 are included in your monthly payments. Any applicable down payment is
added to the initial payment, which is billed at the time of shipment.

Buyer agrees that all payments will be made with payment cards issued by VISA, Master Card, American Express, or Discover, drawn on
banks located within the 50 United States. Buyer agrees and guarantees that the credit line on the credit card designated by Buyer will
continue to be sufficient to permit the card issuer to authorize the debit of each payment as applicable. If that card expires or is
replaced, Buyer agrees to notify Seller and supply an alternate card with the necessary funds available. Such notification shall be
either in writing to Seller’s address at 8 Thornton Rd, Oakland, New Jersey 07436, by email to acctserv@americanmusical.com, or by
telephone to Seller’s Customer Service Department at 1-877-281-8332. Pre-paid debit and credit cards and the PayPal payment option
are not permitted for installment plan orders. You must be at least 18 years old to participate.
If you are under 18, you may use the Installment Payment Plan only with involvement of a parent or guardian.

Buyer agrees that any plan with 8 or more payments is subject to credit approval, which may involve Seller requesting a credit report
from one or more of the credit reporting bureaus. You also agree to allow American Musical Supply Customer Service to contact you
using any information you have provided to us on this order or previous orders in the event that you have not made your scheduled
payment.

Buyer further agrees that, to the extent permitted by applicable law, Seller may impose a fee of $10.00 for any payment, which is not
made on a timely basis.

American Musical Supply reserves the right to report delinquent payments to credit agencies. In the event the purchaser defaults on
this agreement, the purchaser will be responsible for any and all reasonable collection costs, court costs, attorneys' fees, or any other
expenses incurred in the collection of this debt. This installment payment plan offer is governed by the laws of the State of New
Jersey.

Signatures:

Buyer understands and agrees that Buyer’s e-Signature executed in conjunction with the electronic submission of Buyer’s application shall be legally
