CONTRACT OF SERVICE

KNOW ALL MEN BY THESE PRESENTS:

This contract of Service is made, entered and executed this 06th day of JULY
2019 at Barangay Banaba Cerca, Indang, Cavite by and between the following:

Region IV CAVITE AMATURE LEAGUE VOLLEYBALL OFFICIAL, INC.

represented by HYDY MAY R. RODRIN., official point person to manage the
affairs of the association and head of the officiating officials of the Barangay
Volleyball Tournament of Barangay Banaba Cerca, Indang, Cavite, of legal age
with residence at Banaba Cerca, Cavite, herein referred to as the first party;

AND

The Barangay Government of Banaba Cerca, Indang, Cavite represented by

the Punong Barangay EMILIO S. PINPIN, herein referred to as the second party.

WITNESSES

WHEREAS, the SK Chairman of Barangay Banaba Cerca is the organizer of

Barangay Volleyball Tournament at Banaba Cerca, Indang, Cavite

NOW, THEREFORE, both parties have agreed on the following:

That the first party shall conduct sports clinic on the participating teams on
June 29, 2019 before the commencement of the tournament;

That the first party shall provide referees who shall act as officiating officials
during the entire duration of the tournament;

That the second party shall pay the first party the amount of EIGHT
HUNDRED FIFTY PESOS (P 850) per game, the partial payment of which shall
be made upon completion of the elimination round or a total of FORTY SIX
THOUSAND SEVEN HUNDRED FIFTY PESOS (P46,750.00) for 55 games.
Final payment of FOUR THOUSAND PESOS (P4,000) for 4 games shall be made
after the completion of the final round of the tournament as certified by the SK
Chairman of the barangay.

IN WITNESS WHEREOF, the parties here to have signed this contract on the
06th day of JULY 2019 at Indang, Cavite, Philippines.
