Ultimately, an obliterative endarteritis develops, and the ensuing cutaneous and subcutaneous vascular necrosis
leads to localized ischemia and further bacterial proliferation. Rates of fascial destruction as high as 2-3 cm/h
have been described. Infection of superficial perineal fascia (Colles fascia) may spread to the penis and scrotum
via Buck and dartos fascia, or to the anterior abdominal wall via Scarpa fascia, or vice versa. Colles fascia is
attached to the perineal body and urogenital diaphragm posteriorly and to the pubic rami laterally, thus limiting
progression in these directions. Testicular involvement is rare, as the testicular arteries originate directly from
the aorta and thus have a blood supply separate from the affected region.

Fascia
is a layer of fibrous tissue. that permeates the human body. A fascia is a connective tissue that surrounds
muscles, groups of muscles, blood vessels, and nerves, binding those structures together like plastic sandwich
wraps.[2] It consists of several layers: a superficial fascia, a deep fascia, and a subserous (or visceral) fascia and
extends uninterrupted from the head to the tip of the toes.[3]
Function
Fasciae are normally thought of as passive structures that transmit mechanical tension generated by muscular
activities or external forces throughout the body. Some research suggest that fasciae might be able to contract
independently and thus actively influence muscle dynamics.[3]
The function of muscle fasciae is to reduce friction to minimize the reduction of muscular force. In doing so,
fasciae allow muscles to glide over each other.

The deep layer of superficial fascia (Colles' Fascia) is thin, aponeurotic in structure, and of considerable
strength, serving to bind down the muscles of the root of the penis.It separates the skin from the superficial
perineal pouch.
Buck's fascia is a layer of deep fascia covering the penis.It is also known as the "deep fascia of the
penis".Buck's fascia is continuous with the external spermatic fascia in the scrotum and the suspensory ligament
of the penis.The deep dorsal vein of the penis is inside Buck's fascia, but the superficial dorsal veins of the penis
are in the superficial (Dartos) fascia immediately under the skin.
The dartos fascia is a fat-free layer of smooth muscular fiber outside the external spermatic fascia but below the
skin. It is a continuation of Scarpa's fascia, which is a membranous layer of the subcutaneous tissue in the
abdominal wall. The wrinkled (rugose) appearance of the scrotum is due to this layer of fascia.
Scarpa's fascia The deeper, membranous or lamellar part of the subcutaneous tissue of the lower abdominal
wall.
