8-foot Exhaust Fan Shaft Damage Report and Proposal


Baladna Farms Phase II October 2018

Dear Mr. Ibrahim Heyasat,

This report and proposal are in regards to the 8-foot exhaust fan shafts damaged in phase II. The damaging
of the fan shafts is due to the misalignment of the 38-inch fan sheave which weighs 95 pounds and the shaft
itself. If the sheave is not as perpendicular as possible to the shaft a wobble effect will be produced which
will damage the shaft inside the bearing. As close to perfect alignment must be achieved during installation
and maintained through out the duration of the fans life to prevent damage of the fan shaft and/or the
bearing.

However, for the sake of a long-term good relationship with Baladna and UCC, Agpro is prepared to offer the
following proposal to help rectify this situation in hopes of moving forward.

Agpro is willing to provide the following to offer support in bringing the fans back to full operation:

1. On-site support of an Agpro technician for 12 working days at no charge provided accommodations,
transportation and meals are provided at other’s expense.

2. Agpro will send 20 new shafts to start the replacement of worn shafts at no charge provided shipping is
paid for by others.

3. As shafts are replaced, damaged ones will be identified and scrapped after bearings are removed. Agpro
will provide replacement shafts up to 83 additional as per Baladna’s report that 103 shafts are damaged at
no charge.

4. Current shafts that are not damaged will be modified to the same specs as the new shafts with a recessed
indentation in-line with a new set screw and loctited into place.

5. New and reconditioned shafts will be fitted with new setscrews provided by Agpro at no charge.

6. Baladna will replace shafts, align pulleys, torque setscrews to specifications and use Loctite to ensure set
screws remain tight, as specified in rework instructions provided by Agpro.

This proposal will be honored by Agpro for the 103 fans that are reported from Baladna as having damaged
shafts on this situation only.
