LEARNING CONTRACT
Education 15 – Student Teaching

I, Roselle Yarie A. Larrobis, of legal age, currently enrolled in Education 15-Student


Teaching, promise that I will do my best to comply with the requirements of the course and
submit them on time. I also promise to study hard, project myself personally and
professionally, as well take responsibility in all my actions based on the best behavior expected
of a future teacher.

In a situation where I don’t perform the expectations to the highest level, I will not
pressure my professors to change my grades just to have a passing mark. Instead, I will abide
by the rules and regulations of the University on academic requirements and conduct.

Requirements of the course that I need to comply include:

1. Basic requirements to be submitted before deployment :


a. Grade qualification
b. Application for student teaching
c. X-ray results from the hospital
d. Psychological exam from accredited psychiatrist
e. Medical exam from accredited doctors
f. Parent’s waiver
g. Photocopy of insurance
2. Exemplary personal and professional behavior both in-campus and off-campus
(including proper grooming and wearing of the prescribed uniform daily).
3. 90% attendance on scheduled class meetings at Campus II, Bais City
4. At least 500 hours student teaching wherein 360 hours actual teaching for the BEEd
And 140 hours service is expected, and 300 hours actual teaching for the BSEd with
200 hours service. I am also expected to be in my assigned school from 7:15 in the
morning to 5:00 in the afternoon. In case I have overload courses, I will take them
after 5:00 in the afternoon or on weekends.
5. Portfolio to be submitted during the last day of February, 2019.
6. Presentation of a relevant action research with a grade of 85% before mid-February,
2019.
7. 100% attendance to relevant seminars and trainings equivalent to 40 hours.
8. Attendance to bequeathal ceremony/moving up, graduation practices, recollection,
Job Entry Skills Training and other needed performances and activities.

That I fully understand the expectations enumerated above hence, I affix my
