PSYCHOLOGICAL ASSESSMENT REPORT

IDENTIFICATION DETAILS

Age : 06 Years 05 Months 20 Days
Education : 1st Grade
Date : 21 November, 2019

Referred from : School support learning centre


Referred for : To assess the current level of Intelligence and screens SLD

BACKGROUND INFORMATION:
The boy is only issue of non-consanguineous union. Full-Term Forceps Delivery. After
delivery the baby cry immediately and birth weight was 3.40 Kgs. His developmental
milestones were reported to age appropriate except speech. He started schooling at the age of
3 years and currently he is in 1st Standard. He has difficulties in reading and writing. He does
not giving eye contact with strangers for a required period of time. His concentration levels
are very less. He was referred from school for IQ assessment and Screens SLD. The boy’s
parents bring him for detailed psychological assessment. There was no family history of ID,
MI or Epilepsy reported.

TEST ADMINISTERED:

1. Binet Kamat Test of Intelligence (BKT)


2. Vineland Social Maturity Scale (VSMS)
3. NIMHANS-SLD Battery (level-I)
BEHAVIOURAL OBSERVATIONS

The boy was cooperative. Touch with the surroundings. Right handed. He has less eye
contact. Rapport could be established with difficulty. He is following simple instructions. He
knows the names and uses of familiar objects. He can speak sentences and makes
conversations. He is unable to explain relating experiences. He has right and left orientation.
Basic concepts are well developed. He knows similarities and differences. He is unable to tell
greater and less numbers. He is partially dependent for his self-help needs. Restless was
noticed during assessment time.

TEST FINDINGS:

1. On BKT, the boy’s basal age and terminal age are 05 years and 08 years, his Mental
Age (MA) is found to be 05 years 8 months, and his Intelligence Quotient (IQ) was
88, which indicating that the boy has “Below Average Intellectual Ability”.
IQ Range 69 and 70-79 80-89 90-109 110-119 120- 129 130 and
Below Above
Classification Extreme Borderline Low Average High Superior Very
Low Average Average Superior

2. On VSMS his social maturity is 5 years 6 months and social quotient (SQ) 85, which
indicates his social adaptive function are not appropriate to his age level.

3. On NIMHANS Battery (Level-I) for learning disability, his ability to focus and
sustain attention on a task was found to be inadequate.
Assessment for Visual Discrimination performance showed he is able to
discriminate in a given task.
Her Visual Memory skills were found to be inappropriate to his age level.
On the Auditory Discrimination test, he has adequate identification for same and
different word pairs. He needs repetition at times.
Auditory memory was found to be inadequate. His speech and language
expression was inadequate to his age and he speaks with low tone. On Visual Motor
Skills he found to be inadequate
On copying task performance found to be significantly below his grade level. He
confuses with capital and small letters and also mirror writing noticed. He was
unable to write before and after numbers. He took long time to complete his task.
IMPRESSION: Based on current clinical observation, interview and test results the boy has
obtained the IQ score of 88 (± 5) indicating that Master M’s Intelligence is Below
Average with specific learning disability.

RECOMMENDATIONS:

 Discussed with parents regarding test results and clinical impression


 Remedial Training
 Attention Enhancement Training
 Regular inputs towards his academic performance
 Review with School support learning Team
