Duties and responsibilities of pharmacists (pharmacists)

Pharmacists are part of a team of professional health services working in a


pharmacy, whether in the hospital pharmacy or the pharmaceutical industry. Focusing
on the effectiveness and safety of drug use, a pharmacist has the duty to distribute
medicines.

The pharmacist's duty is not only responsible for administering drugs


prescribed by doctors but as a pharmacist also needs to monitor or examine the drug
before it is given to patients regarding drug administration, pharmacy service
standards, prescription review; (including administration, pharmacetic suitability and
clinical considerations, dispensing , Drug Information Services (PIO), counseling,
Home Pharmacy services (home pharmacy care), Drug Therapy Monitoring (PTO),
and Drug Side Effects Monitoring (MESO).

Lately, there are often errors or negligence of the pharmacist in carrying out
their duties as the person in charge of medicine. like the case of a patient named
Loretta Macpherson (65) with brain cancer died in St. Charles Medical center,
Oregon USA. He was declared dead due to mismanagement of drugs by medical
personnel. Seizure medication that should have been given to Loretta by medics was
not given. Due to negligence of the medical officer he was even given a paralytic
drug. Mistakes were made by pharmacists who were supposed to put phospheniton in
IV bags as a seizure reduction according to the prescription given by the doctor. The
pharmaceutical worker instead injects a paralytic drug called rocuronium.

In addition to the above cases many other cases that occur due to errors in
drug administration such as the case of Mr. KY 58 years old is a patient at the health
center complaining of sore and red eyes because of being exposed to sand grains
when using a motorcycle on May 2, 2017 came to the doctor and given a prescription.
at home the patient just reads that the drops given in writing are chlorampenicol 3%
ear drops but the patient thinks the drug may be used for eye drops and ear drops
when used the patient's eyes feel more sore.

The patient then comes back to the doctor at the health center and complains
about the medicine given, the doctor changes the prescription but it turns out that
when he gets back to reading the medicine is another ear drops the patient still thinks
it can be used for eye and ear drops but when the patient's eye drops the eye is even
more sore and pain and dizziness until the patient goes to the eye specialist with his
family, after examination the patient's eyes are still normal but can not be ascertained
for the future and this makes the patient very uncomfortable and finally protest
against the Puskesmas so that similar incidents do not occur.

There are so many mistakes in drug administration that make many people
lose their lives, from the above cases it can be concluded that the need for
cooperation among fellow health workers, and accuracy in carrying out tasks, and can
be seen how important our role and responsibility are in bearing the name as a
pharmacist. Error in giving the dose until the drug will have a very fatal effect,
therefore do not consider the role of the pharmacist only as a compounding drug or
commonly known as a drug seller. And it needs good communication between health
workers because familiarity is needed to break the rigid atmosphere, and useful to
make it easier to solve various problems on the ward. To establish intimacy, good and
sympathetic communication is needed, a lot of experience, the need for confidence
because lack of confidence is the number one killer who will slowly turn off a
pharmaceutical career.
